While much of His Lie in April does take place in the Nerima ward of Tokyo, like the original story, there are actually several places in this new story that take place in locations outside of that area. Here they are.
CHAPTER 8 - STAND BY ME, KAORI (also mentioned in CHAPTER 34: A HEART-TO-HEART TALK, and CHAPTER 52: THE WIND BENEATH OUR WINGS (II))
MURPHY FRIES (Real Life location: Robson Fries, website: robsonfries.com)

Believe it or not, while Murphy Fries is merely fictional, it is based on the real life location called Robson Fries, located in the Setagaya City section of Tokyo. A description @ bento.com:
"Poutine - that peculiar Canadian version on french fries served with gravy and cheese - is the exotic specialty at this small take-out shop. They're served with toppings ranging from fried onions and garlic to meatballs and roast beef, and you can also opt for variations featuring chili and butter-chicken curry instead of just gravy. Chicken wings are available as a side dish - Buffalo, Cajun or BBQ style. There's also a selection of drinks and, depending on the hour, the shop may be configured as a standing-only bar/restaurant."
The restaurant is 6 miles/10 km south oh Nerima City, and it actually takes 45 minutes to get there by car! It also takes the same length to get there via Tokyo's subway system.

CHAPTER 34 - A HEART-TO-HEART TALK/THE BEST OF FRIENDS

This aquarium, where Kaori and Hiroko talk about Kousei in chapter 34, is actually part of a larger complex called Sunshine City. It comprises of a shopping mall, corporate offices and hotel rooms. The aquarium itself is located on the upper three floors of the complex.
The aquarium is 5 miles/9 km from Nerima City (making this the closest place from Nerima in the story); it takes about 26 minutes to get there by car, or 28 by transit.

CHAPTER 27: TWO SINKING HEARTS/CHAPTER 41: A LOST SOUL/METAPHORICAL TRIGGER

After being hit on the head for the second (on a metal lamp post) and third times (on a concrete curb), Kousei Arima is taken to this clinic for examination on those blows to the head, and for possible brain damage.
The clinic is 10 miles/17 km from Nerima City (this is the farthest location in the story). It takes 44 minutes to get there by car, or 48 minutes by transit.

CHAPTER 55: A CHANCE ENCOUNTER, A NEW BEGINNING

Unlike the original story, Kaori sends herself to a different hospital, and after she was stopped from committing suicide in the same method that ended Kousei's life, she voluntarily checks herself here, and meets Keiko Tamura.
The clinic is located 7 miles/11 km from Nerima City. It takes 39 minutes to get there by car, or 50 minutes via transit.
