"Gabby? Can you hear me?" I smiled, looking in the rear view mirror. I could see her holding her chest and struggling to breathe again. "Gabby listen to me carefully because I know you can do this. Insert a large-bore 14-gauge or 16-gauge needle with a catheter into the second intercostal space, just superior to the third rib at the midclavicularline, 1-2 cm from the sternal edge. Use a 3-6 cm long needle, and hold it perpendicular to the chest wall when inserting. Once the needle is in the pleural space, listen for the hissing sound of air escaping. Remove the needle leaving the catheter in place. The cannulas left open to air. Secure the catheter in place. Prepare the yourself for the tube thoracotomy." I sighed trying to speed up to get her to hospital. I listened carefully and then heard her breathing get worse.
"Gabby you need to do this in the next few minutes otherwise your going to die." I sighed raising my voice. I radioed in.
"Ambulance 61 to main."
"Go ahead. Over."
"I'm driving my partner to med but she's currently needing a tube thoracotomy but she's performing it on herself because I'm driving. I need some assistance. If I pull over and begin the surgery can I get some back up to help me?"
"Firehouse 51, Matthew Casey here I'm on my way to Brett and Dawson now with Kelly Severide now." Matt radioed back. I pulled over with sirens and lights on. I climbed into the back and Gabby was laying on the bed with the needle and everything halfway in but she was covered in blood and fitting. I began trying to get on with the decompression but it wasn't working because she was fitting so harshly. It didn't take long before I heard the sirens of the fire engine. Then Matt and Kelly jumped out and onto the ambulance.
"What the earth happened?" Matt smiled.
"I was driving so she attempted the decompression on her own. Clearly as she's done it. She's fit the wrong spot. Started having a fit and caught a artery and now I need her to get to med as soon as possible. So if I drive can you both work on doing this?" I smiled hopping back into the front. Otis took over driving the fire engine as the lads got to work.

"Brett tell me what I need to do?"
"Right Matt grab the needle off the table. Find the 2nd rib and move to the right about 5cm and don't even think about it. Just stab the needle right into the lung between the 2nd and the 3rd rib." Kelly was holding her head and holding her down on the table while I got this needle in her ribs and attached the catheter on the end of the needle. A massive gush of air come out and her chest began rising and falling properly while she continued to have a seizure.
"Brett I did it. She's breathing properly but this seizure is still happening." I sighed.
"That's fine. Just don't let her fall off the bed. Don't let her head hit anything. Hold her still and carefully until it stops." She smiled as we pulled into Med. I helped get her bed off the ambulance and into the hospital.
"29 year old female, crush injury, right side lung decompression performed en route to Med. She's had a seizure for 18 minutes now on the way to Med." I sighed as they took over. I stood at the door with the others while we waited. It literally take seconds and 1 injection before she was out of the seizure. The nurse come out and told us we could go in. When I went in Gabby smiled and hugged me straight away.
"Thank you so much for saving me." She smiled beginning to cry into my shoulder.
"That's okay. Don't attempt a lung decompression on yourself ever again." I laughed. I stayed with her for 2 hours until her blood pressure got up and then we went back to the firehouse. She used my arm as support because her chest was hurting and it was making her tired. We sat down at the table and she smiled at me.
