Season 1
Before the main story we'd get some introductions episodes showcasing that the Sonic Prime universe is different from the main continuity. The opening of the show plays out the same as canon with Sonic and his friends fighting Eggman and them breaking the Paradox Prism during their fight
Sonic then wakes up and finds himself still in Green Hill but all of the colour has been taken away. He then hears Shadow who scolds him for what he's done
After arguing for a bit they see a shard of the Paradox Prism. It opens a portal and the 2 hedgehogs reluctantly put aside their differences and go after the paradox pieces
When they arrive in new yoke city, Sonic and Shadow find Tails AKA Nine and Sonic tries talking to him. This is when Nine gives Sonic his new shoes and gloves which will act as a detector for the next shard piece
While the trio make their way to next shard their captured by Rusty Rose and her forces and are taken to the chaos council. After a similar escape they made in the show, Sonic, Shadow and Nine are ambushed by Egg robots but are quickly saved by the resistance members Rebel and Knucks
After going back to the resistance Sonic and Shadow learn from Nine that they have to go across other dimensions to retrieve the rest of the shards and bind them back together but their not far behind since the Chaos Council is after them too
Nine then takes the Prism to a world called the Grim which serves as a safe place for getting the shards
Along their adventure Sonic and Shadow would meet many different versions of themselves and their friends not just the ones from the show but even universes like Sonic Boom, Sonika the Hedgehog, Human Sonic, Pirate Sonic, Space Fighter Sonic and etc
After gathering as many shards as possible Prime Sonic decides to give them to Nine who becomes all powerful and betrays him and the resistance. He kills the Chaos Council by blowing them up and and declares himself to be all powerful
Season 2
Prime Sonic and Shadow assemble a team of of alternate versions of themselves and their friends from other worlds to fight against Nine and his army. In the final battle 9 Prime Sonic uses his Prism form to fight Nine
However Nine destroys himself after abusing his own power too much. In the end he was defeated and all the heroes went back their home worlds
It can also be assumed that Prime Sonic may or may not of asked out Shadow after all of that was over (mostly because I like Sonadow in Sonic Prime)
