Ascend Hospice

Founded in 1995, Ascend Hospice is a nondenominational, multicultural hospice and palliative care provider built on the belief that hospice services should reflect the life we live – one lived fully and with dignity. With offices in New Jersey, Massachusetts, Pennsylvania, Kansas, Missouri, California and Virginia, we provide care for approximately 850 patients in nursing homes, assisted-living facilities, hospitals and private homes. HOLISTIC THERAPIES Ascend Hospice believes in using a personalized and holistic approach to hospice care. From aromatherapy and Reiki to pet therapy and massage, we incorporate these specialized therapies to enhance the hospice experience for all of our patients. SPECIALIZED HOSPICE PROGRAMS At Ascend Hospice we are committed to delivering exceptional and compassionate end-of-life care for patients and their families, offering a wide variety of specialty programs including the Essence of Journey, Essence of the Heart, Veterans Program and PATH program. LEVELS OF CARE At Ascend Hospice, our interdisciplinary teams provide four levels of care based on the individual needs of the patient and family. Routine: provided in the home or in a healthcare setting. Respite: short-term relief for caregivers by placing the patient in a nursing facility for up to five days. Inpatient: a more intensive level of care designed for short-term pain and symptom management; provided in a hospital or skilled nursing facility. Continuous Care: one-on-one skilled nursing care designed for short-term crisis intervention involving pain and symptom management. BEREAVEMENT COUNSELING SERVICES We understand that the grieving process is different for every individual – even among family members. Ascend Hospice offers counseling services that begin at the time of admission and bereavement assistance that continues for 13 months following the patient’s death.
