Integration of gerontology expertise into a primary care team in family medical practices can improve care of older patients; support struggling caregiving patients; and expand the effectiveness of social prescribing for aging patients and their families. Creative Aging Company currently delivers it’s gerontology sessions through an innovative partnership with a local family medical practice utilizing a Social Prescribing framework. Social Prescribing is a holistic approach to healthcare that bridges the gap between clinical care and social care that provides a way for healthcare providers (doctors, nurse practitioners and other healthcare providers) to formally refer patients to non-clinical programs and services aimed to improve overall health and well-being. Offering support services to older adults and caregivers through this unique collaboration between primary care and gerontology has created a more holistic and preventative approach to patient care, reduced non-medical visits to address aging issues and minimized reliance on medical care. During National Primary Care Week, we recognize the importance of primary care physicians, our first resource for a healthier life. They are the family practice, general practice, pediatricians, and internists who become familiar with our family history, lifestyle, and everyday needs. Many of us spend years with the same PCP, relying on them to provide various care. While they often perform routine health screenings, they also treat minor illnesses and chronic conditions.
