🚨 New ACTION publication 🚨 Using data from the ACTION registry, authors reviewed outcomes for patients who received Impella pumps alongside VA-ECMO from 2014 to 2021. The results? Out of 20 patients, 45% recovered, 40% moved to durable VADs, and 5% got heart transplants. These findings suggest Impella support could be vital for older pediatric patients to help their hearts recover or prepare for the next steps in treatment. Delighted that our work, lead by my colleague Jared Wohlgemut, on the accuracy of pre-hospital clinicians to detect major haemorrhage has been published in TSACO. I think we tend to assume that experienced clinicians have excellent diagnostic skills, even in difficult situations. This study, along with our previously published work on the detection of life and limb-threatening injuries (https://lnkd.in/eR4YjRqY), quantifies just how good (or indeed, not so good) we are at this. Only 70% of major haemorrhage cases, defined by receiving at least 3 units of PRBCs in under 60 mins within the first day of admission, were detected by pre-hospital clinicians - penetrating injury and abdominal trauma made this more likely to be missed. Whilst we should commend the 94% specificity of clinicians, ensuring minimal overtriage and therefore limiting trauma team fatigue, the delay in recognising major haemorrhage has a significant impact on patient outcomes - in this study we found that non-detection of major haemorrhage leads to a 3-times increase in mortality. Recognising the limitations of our diagnostic accuracy as clinicians is key to being able to understand how to support injury identification and decision-making in trauma to improve outcomes for our patients.
