An ambitious model of care redesign for low-acuity patients in our ED has reduced length of stay by 25% for 85 patients per day. Using improvement methodologies, engagement with frontline clinicians, multidisciplinary involvement and lean management principles we have rebranded our Ambulatory Care stream as Fast Track, and created a novel short-term admission unit called Rapid Stay. The Fast Track model is defined by specific exclusion criteria that aims to keep the space lean, batch low complexity work, allow for early senior decision-making, reduce unnecessary testing and improve patient safety. Patients identified as “low acuity” after two layers of risk stratification (triage senior clinician review OR triage comprehensive assessment), who need 1-8 hours of care and who are likely to be discharged home can be admitted to Rapid Stay, an 8-bed ultrashort-term admission unit. The new models have stood up well against recent system pressures and have exceeded initial expectations. Feedback from patients and staff has been overwhelmingly positive. Another ‘Code Yellow’ has been called for the Royal Darwin and Palmerston Hospital. How can the Chief Minister continue to hold the position of Health Minister? Under normal rules, it is three strikes and you’re out! Today’s ‘Code Yellow’ comes on top of a further thirteen ‘Code Yellows’ at the Royal Darwin Hospital over the last couple of years with shut-downs in February 2021, August 2021, two in September 2021, February 2022, August 2022, November 2022, March 2023, May 2023, June 2023, two in August 2023, and again in September 2023. A number of elective surgeries have been temporarily postponed so that patients with more urgent care needs can be admitted to hospital. The Chief Minister’s performance is nothing short of a disgrace and she needs to step aside as the Health Minister. On top of this, the Chief Minister has ignored the urgent need for renal dialysis facilities in Elliott, Canteen Creek, and in Ti Tree, while additional chairs and beds are needed at Tennant Creek, Borroloola, Ali Curung and in Alpurrurulam. Chief Minister, the NT Health system is in crisis! It is time to stop the chaos and dysfunction and start delivering for all Territorians. #BoundlessIncompetence
