Fentanyl is dosed in micrograms, which illegal manufacturers would have difficulty measuring. It requires tremendous dilution, like 5000 to 1 to get a very high 48-72 hour timed release dose. That dose would need to be divided by 72 to get a one hour high dose. Then that dose needs to be divided by 8 to get an initial hour dose for individuals who never used fentanyl or other high dose opioids before. I doubt that the manufacturers & distributors of illegal fentanyl would bother to do the calculations or even verify the purity of their drug.

I come to this subject as a healthcare writer and subject matter expert on US public health policy for regulation of prescription opioid pain relievers and of clinicians who employ them in managing severe pain. I'm easy to find in a google search for "Richard Lawhern" and I don't mind being publicly visible. We now know beyond any reasonable contradiction that the US CDC acted in bad faith from a pre-existing anti-opioid political agenda rather than from science, when they issued their now-infamous opioid prescribing "guidelines" in 2016 and 2022 . CDC's errors were repeated by the Veterans Administration and US DoD, both of whom issued blanket policies denying safe and effective pain medications to millions of patients. Pain patients are dying all over America, deserted by doctors and pharmacists afraid of persecution by DEA or State Medical Boards anddriven by the CDC's fatally flawed guidelines. It is time for Congress to act: CDC and the VA must be removed from all policy making roles in public health practice for management of pain. Likewise, many elements of the Controlled Substances Act of 1970 must be repealed or seriously revised. Failing to control illegal drugs coming across our borders the DEA is using Gestapo tactics and outright fraud to persecute doctors with false testimony and pre-trial asset confiscations designed to deny them legal due process. I couldn't agree more with Cal Cates about dismantling the preconceptions regarding any and all folks who present with a larger body size. Didja'll know August was Fat Liberation Month? I'm a bit late, but I wanted to recommend this fantastic class that our our dear and deeply missed friend, Amelia Mitchell, created for Healwell. In this class, Amelia shares her own experiences as both a patient and a provider and dismantles the assumptions and generalizations (both implicit and explicit) many of us make about our patients and clients that lead to real and lasting harm. I cannot recommend this class enough.
