Device reps will be more useful in the ASC setting to ensure any special needs are accounted for. The trouble is the downward pressure on costs in the ASC. I have spoken to two distributors in the Chicagoland area who loathe negotiating with ASCs trying to make Medicare knees feasible because CMS is underfunding outpatient joints at sites distant from the hospital. Theoretically one could see a future where Optum (aka UnitedHealthcare) buys a small implant company for use by the surgeons it employs in the ASCs it owns. That’s the only way to make money off of Medicaid/Affordable Care Act joints - in the hospital or ASC. There would have to be massive vertical integration from implant manufacturer to surgeon to hospital to insurance company. I would be hesitant to be a part of; hopefully antitrust laws ensure that will ever happen in the US.
