Matt Harless you are so on point! I agree that leadership in our hospitals must be held accountable for the quality of patient care AND that it is time to rethink EVERYTHING we have ever done or considered "normal" in nursing. Why does the brunt of the patient experience and quality of patient care fall on nurses when facility leadership has the opportunity - I mean responsibility - to participate in improving them both by being a part of the solution? Your statement that "elective surgeries continue" makes us all wonder how they can be scheduled? Who is going to staff them if we already don't have enough nurses to care for the patients in the hospital? Oh, I see. It's about revenue. Don't get me wrong. As a business owner I get that to stay in business a business must turn a profit... even the non-profits. That's a topic for another day. But here's the thing... How can we ever consider today's profitable hospitals successful when they are not adequately staffed? When their nurses are burned out? When they are turning over 100% of their staff every 5 years? When the patient experience is poor? When patients are waiting days in the ER for a bed? When adverse events and errors occur? If profitability is the only measure of success, then, I guess we are all good. But when I hear from nurses daily who share their stories of fear, depression and anxiety experienced 24 hours before their next shift; extreme burnout, fatigue and exhaustion; and increasing nurse suicides, I don't see success. I see instead a very broken healthcare system digging in its heels to continue carrying on as we have always done. Why do bedside nurses leave?… We are down three nurses today, but at least we have one extra tech? I ask, who in the hospital has an active nursing license but is not being asked to help the bedside nurses out? If “we” are all in this together then why are there so many hospital employed registered nurses exempt from helping out in a crisis? Does anyone call the registry anymore? All of the elective surgeries are still scheduled! The ED is not on diversion. Why would this not qualify as community emergency? Am I wrong that hospitals receive grant money to be a community resource in times of need? Should patients expect to spend days in the ED waiting for a bed? Should I vasicilate from one week being called off because I am not needed, to the next week being “asked” to do the work of two and even three people? At the Harless Method, I am committed to transparent honesty with my patients, my coworkers, my leadership and most importantly with myself. I will attempt to do my best. I will accept that I cannot give my patients the time and attention they deserve, but I will not accept this as the new norm. The group of charge nurses working on the staffing panel for the hospital are still waiting for night shift call offs, but no one is looking beyond the problem of staffing and acuity to offer any kind of assistance. Why are hospitals required to have internal and external codes for emergencies in the community or hospital setting, but there is a blatant disregard for emergency staffing levels. This is why I believe abuse, neglect and exploitation charges should be filed in these cases. Leadership teams need to be held accountable when they decide to overseer unhealthy environments while they watch their profit margins soar. #lovingdisruption Thank you for reading my post.
