I will never forget when the medical group I was a part of was purchased by a hospital system the first email stated "Dear employed physician......" I knew in an instant the team based mentality was on the way out, and sure enough the autocratic process began - destroying a collaborative culture that had taken over a decade to build. With the number of administrators going up at a rate 10X that of doctors in the last 20 years, and with over half of docs now being employed by hospitals and/or private equity this is a growing issue. Only doctors can determine where medicine can be made more efficient and safe medically, and the ever increasing demand for administrative work on doctors is burying them and reducing the opportunity by demanding they become HCC and gaps in care closers rather than increase their trust and effectiveness with patients. As this articles states: "A mindset shift from administrative to collaborative clinical leadership, with physicians playing a more central role in the workflows realigns appropriate incentives and workflows." It points out the error of reducing clinical input and expanding administrative control and oversight. Despite this, the concept of physicians as leaders has been overlooked in healthcare, despite patients recognizing them as the arbiters of quality care and treatment decisions. The Medical Association has long advocated for doctors worldwide to embrace leadership roles. However, the challenge for young doctors lies in understanding how to wield this leadership effectively. Dr. Oluga suggests that the first step is to become advocates for patients, ensuring they receive the best possible care. Additionally, doctors should advocate for favorable working conditions for their teams, comprising not only doctors but also nurses and support staff. Patient advocacy, he argues, is the pinnacle of healthcare leadership. He urges doctors to approach leadership roles with a clear understanding of the challenges facing healthcare delivery, emphasizing the importance of policies that make sense both for patients and healthcare providers. Ultimately, Dr. Oluga's message resonates with the need for doctors to embrace leadership at every stage of their career, from medical school onward. Leadership, he emphasizes, is not merely a title but a responsibility to advocate for patients and improve healthcare delivery. The delivery of clinical care is based on research, but the NHS spends on leadership development without clear impact on patient outcomes. Collective leadership is crucial at all levels in healthcare. Nurse leaders should be participative and emotionally intelligent. Medical engagement is linked to better performance. Effective team working is essential for success. Leadership, culture, and climate impact patient outcomes. Evidence-based leadership development is needed. National leadership should support and enable high-quality patient care. Developing leadership strategies based on robust theory is crucial for healthcare organizations.
