Last week we celebrated a major milestone for affordable prescription care for Indiana’s most vulnerable. In conjunction with our specialty pharmacy partner, AndHealth, we hosted an official ribbon cutting for the JPCHC Pharmacy. Now, local residents and patients of JPCHC primary care across Indiana have access to low-cost retail, prescription, and specialty medications, diabetic testing supplies, over the counter medications and vitamins, and more. Special thanks to Senator Andrea Hunley, Representative Blake Johnson, Deputy Mayor Judith Thomas, AndHealth CEO Matt Scantland, and JPCHC CEO Marc Hackett for their impactful remarks. The JPCHC Pharmacy accepts all Medicaid plans and works with the federal 340B drug program and manufacturers to get the best prices possible for the community. Because JPCHC is a nonprofit, the pharmacy is able to provide much better service than big box retailers, taking the time to help healthcare providers and patients find the best solutions at the best price possible. As we continue our fight for PBM reform, awareness plays a key part in our success. As a patient, I've seen firsthand the intricate dynamics of Pharmacy Benefit Managers (PBMs) and their impact on healthcare accessibility. Today, I want to shed light on this often overlooked aspect of our healthcare system and why it's crucial for us to advocate for fairness, especially for pharmacies. PBMs play a significant role in our healthcare journey, acting as intermediaries between pharmacies, insurance companies, and drug manufacturers. While their purpose is to streamline processes and negotiate drug prices, the reality is often far from ideal. One of the most glaring issues I've encountered is the lack of transparency surrounding PBM practices. Patients are often left in the dark about how PBMs determine drug pricing and reimbursement rates for pharmacies. This opacity can lead to inflated drug costs, limited access to medication, and financial strain on both patients and pharmacies. Moreover, PBMs' rebate and reimbursement models have been criticized for favoring larger pharmacy chains at the expense of independent pharmacies. This imbalance not only threatens the existence of local pharmacies but also limits patients' choices and access to personalized care. It's time for us, as patients, to recognize our role in advocating for fairness within the healthcare system. By demanding transparency and accountability from PBMs, we can ensure that pharmacies are reimbursed fairly for the vital services they provide. Supporting local pharmacies isn't just about convenience—it's about preserving access to quality care and fostering a more equitable healthcare landscape for all. Let's use our voices to drive change and champion fairness for pharmacies. Independent pharmacies continue to be crushed by the monopolistic big 3 PBM’s. If their unfair practices are allowed to continue the effect on our healthcare system will be devastating. Plain and simple, patients deserve choice. Having access to their local independent pharmacy where they receive significantly better care then the big box retail pharmacies is paramount. Work with a PBM that is not beholden to Wall Street or Private equity and being independent themselves are huge supporters of locally owned pharmacies. You’ve likely seen the announcement from CMS that they will, for the first time, negotiate drug prices for Part D ( meds you pick up at the pharmacy ). The very first drug listed is Eliquis. This is most commonly prescribed to prevent blood clots when someone has atrial fibrillation… a very prevalent condition in the Medicare population. For seniors on a fixed income, they have difficulty affording it. Here’s how it shows up for how healthcare utilization occurs. An auth request is submitted for a Watchman procedure. The rationale is SUPPOSED TO BE because they can’t take drugs like Eliquis because they are a high risk for falls, maybe have had internal bleeding, etc. (A Watchman fills a part of one’s atria where, with AFib, clots can form so by filling it, the risk is reduced such that a drug like Eliquis can be stopped) But what I’m seeing with increased frequency is that they can’t AFFORD the medication so they have been referred for an invasive (and costly) procedure. Seniors being able to afford care is real. Not being able to afford care drives up cost, whether it’s substituting a more costly procedure ( where co-pay is manageable for them ), or ER visits because they couldn’t afford transportation to dialysis, or an admission because they had to ration their insulin. Newsflash: we all pay for the cost of healthcare… consumer, employer, tax payer.
