The use of AI driven precision medicine is the wave of the future for identifying bio markers that increase the chance for succesful clinical trials in oncology. This technology is ideal when looking into a new class of immune checkpoint targets called Siglec receptors which have many ideal currently available bio markers in cancer such CA-19 in pancreatic cancer ( CA-19 is Sialyl Lewis A , a know ligand for Siglec checkpoint receptors)

Striking fact: biomarker-led clinical trials have up to a 12 times higher chance of success compared to trials without precision biomarkers (ref. in the comments). One of the defining moments in a biomarker-led therapeutic revolution was the 1998 approval by the FDA of the monoclonal antibody trastuzumab (Herceptin). Designed to target the HER2 protein, it was used to treat HER2-positive metastatic breast cancer, either as a first-line therapy in combination with other drugs, or as a standalone second-line treatment. This represented a significant stride forward in targeted cancer therapy, and opened the floodgates for similar advances. Today, we have a plethora of HER2-targeting drugs, including Tyrosine Kinase Inhibitors (TKIs) such as neratinib, lapatinib, and tucatinib, which have broadened the therapeutic scope beyond breast cancer to other cancers that overexpress the HER2 protein or exhibit HER2 gene amplification. Biomarker discovery plays a pivotal role in advancing small molecule #drugdiscovery by identifying key indicators that can aid in disease diagnosis, prognosis, and treatment. These biomarkers, often found in various biological samples, offer valuable insights into disease mechanisms and potential #therapeutic targets, biomarker discovery is a critical component of small molecule drug development, offering a bridge between basic research and clinical applications. By identifying specific molecules or genetic signatures that correlate with disease states, biomarker research provides a roadmap for the development of targeted therapies. These biomarkers may be present in blood, tissues, or other biological samples and can serve multiple purposes, including early disease detection( #diagnostic), patient stratification( #prognostic) and monitoring treatment responses( #therapeuticdrugmonitiring ). The integration of biomarker data into drug discovery pipelines accelerates the identification of promising drug candidates and enhances their clinical success rate. An oldie but a goodie. Tracking of the BCR-ABL fusion during the development of imatinib was a fabulous example of a successful pharmacodynamic biomarker. Imatinib, a tyrosine kinase inhibitor, was developed for treating CML, driven by the BCR-ABL fusion protein. PD biomarkers were crucial in its clinical trials to demonstrate the drug's efficacy and guide treatment strategies. The Philadelphia chromosome was tracked using two methods: 1. BCR-ABL Transcript Levels: A decrease in BCR-ABL mRNA levels by qPCR indicated effective inhibition of the BCR-ABL kinase, correlating with reduced leukemic cell proliferation. 2. Cytogenetic Response: Cytogenetic analysis of Philadelphia chromosome-positive cells. Reduction in these cells demonstrated the drug's impact on the leukemic cell population. The latter was one of the first tests I did when I started my Cytogenetics career in 2005. It works by binding to and promoting the degradation of messenger RNA (mRNA) molecules, which are the templates used to produce proteins. It's always amazing to see innovative, groundbreaking research in the field of cancer treatments and research. As outlined here, the "protein sandwich" technique is offering promising prospects for advancing cancer treatment modalities. Using an “intramolecular bivalent glue” researchers were able to bind together proteins that would otherwise fall apart. This has been crucial in creating more comprehensive, holistic drug development and therapeutic interventions.
