A moving article about the current physician shortage in New Hampshire, especially within primary care. Similar situations are occurring across the country. "Medical practices are full, not taking new patients or putting everyone on a waitlist. Health care systems and outpatient practices are using more nurse practitioners and physician assistants than ever to diagnose and treat a wider variety of conditions, reserving the few doctors on staff for the most complex or serious ailments." This problem is solvable. Here’s how: Scrap roster targets. No one cares. Patients don’t care about roster size. Patients care about access. Roster size is meaningless if patients can’t access care when they need it. Scheduling is a science that requires a scientific approach. #3NA or “Third Next Available” appointment is a scientific measurement of how long it takes patients to access providers when they want to schedule an appointment. ⏱️ A good 3NA metric indicates there is a good balance between demand for providers and provider capacity. But as with any scientific model, education, standardization, measurement, and reporting tools are needed to successfully implement. So, instead of rewarding doctors for high roster counts, we should: a) give them 3NA scheduling tools, training, and targets b) pay bonuses when they hit 🎯 Could it be this simple? 💡Curious to hear about your experience with 3NA (it’s old news). Personal relationships are especially important in virtual care, and this recent JAMA, Journal of the American Medical Association study is more evidence. The study found that remote patient monitoring works better for minority groups when it is accompanied by nurse phone calls. Seeing a provider virtually should not mean sacrificing the relationships you build when you’re in-person.
