Another issue related to discounts is that evaluation models (ie the dreaded spreadsheet) rely heavily on discounts. Also, these spreadsheets use incumbent data and then apply each bidders discounts to that data. As you express with your milk analogy, if I am preferring less expensive milk that doesn’t require any kind of discount, I will show very poorly in the current evaluation process. The Big PBMs know this, which is why they have no incentive to change.
