No Ray of Sunshine Why routine chest x-rays? A quick check of reasonable estimates of sensitivity and specificity ( Samuel VJ, Gibikote S, Kirupakaran H. Ind J Radiol Imaging 2016; Parikh R, Mathai A, Parikh S, et al. Indian J Ophthalmol 2008) : sensitivity 0.5 approximately , specificity 0.996; not a bad screening test as a good screening test has sensitivity specificity = 1.5 or greater (Power M, Fell G, Wright M BMJ Evidence-Based Medicine 2012.) The risk of #cancer associated with a routine CXR is lower than the risk of death driving to the x-ray facility. A wonderful summary of risk and exposure was published a few years ago (Verdun FR, Bochud F, Gudinchet F, et al. RSNA 2008) with a nice follow-on a few years later ( Lin EC. Mayo CLin Proc 2010). An aside, a single #CXR is equivalent to 10 days of ambient radiation exposure. The cost of a chest x-ray is between $230 - $600 (US). (https://lnkd.in/gq3iA4M4) For ease, let us put the value at $400. CXR of 4113 persons resulted in 7 with clinically significant findings. Alternatively, 4113*400/7 = $411,300.00 for one significant finding. Hi everyone Hope everyone is good health A very nice case has been diagnosed this morning. Although, everyone is aware of lung carcinoma but this one is different. A 26 year old was referred to us by pulmonologist because of presence of a well demarcated solid nodule of 4.1 cm in diameter that was enhanced on CT scan in the cardio phrenic angle . We suspected it to be a mediastinal nodule, such as thymoma. A systemic CT examination revealed no tumors other than this tumor. The lung was deflated to confirm the nodule. The histopathological findings revealed a well demarcated nodular lesion composed of proliferation of spindle cells, admixed with some pleomorphic cells arranged in a fascicular fashion with ropey collagen fibers associated with variably dilated blood vessels exhibiting staghorn like appearance, involving the pleural tissue and the pulmonary alveolar tissue. Mitotic figures were few and Immunohistochemically shows positive for CD34 , bcl-2 and STAT6, whereas alpha-SMA, desmin, and S-100 were negative. Based on above facts and as per recent WHO Classification Of Lung Tumors the diagnosis of fibrous tumor of the lung was given. karyotyping was done which revealed very unusual presentation revealing 48,X,−Y, 7, 12, 17 #karyotying #deflation #of #the #lungs #to #determine #acccurate #diagnosis #stapling #device #biopsy #chemoradiation #pleurodesis
