Mosaic’s well developed and integrated person centered processes colour and shape what the person as a client and family experience, our frontline care teams (many whom have been with us for 10 years or more), and our client services (similarly) and community engagement teams all the way up to senior management. The standard model is however typically task orientated, siloed, and medically focused, emphasizing incapacity rather than intrinsic capacity. Much of care today has little time for addressing social emotional needs, hobbies, interests, community, social connections and human meaning. We believe it is a dangerous marketplace that assumes care is the same and all care is basic. Differentiating the Mosaic model from the standard service model is becoming more and more difficult as more and more organizations incorporate person centered terminology in their communications. The other day I overheard a health care professional commenting on how Mosaic was no different from any other care provider in the community. The inference appeared to be that words were cheap. Fortunately, I was able to spend time explaining our differences and reversed this opinion. With respect to our community outreach, we would like to thank those who likewise reach out to us: to those who participate, attend, and enrich our programs, to those who collaborate and invest in the community, to those who volunteer and sponsor. This bigger frame enriches us all and informs and underpins our lives and our model of care.

In their new Forefront article, Evelyne Kane, Victor Murray, and Miguel Rodriguez of the Camden Coalition; Tanya Shah; Alayna Tillman of the University of California, San Francisco LACRC; Alyce Luna; Connie Montgomery of the National Bleeding Disorders Foundation; Lauren Pelley; and Olivia Richard share recommendations for helping Dual Eligible Special Needs Plans (D-SNPs) navigate misalignments with their enrollee advisory committees, from the perspective of people with lived experience who have worked closely with the Camden Coalition—and other organizations—on multiple community engagement initiatives. "In our experience, good community engagement should reflect the community’s values and interests. As members of the Camden Coalition’s community engagement staff (Kane, Murray) and National Consumer Scholars program (Tillman, Luna, Montgomery, Pelley, Rodriguez, Richard), we are committed to building authentic partnerships between health care organizations and people with lived experience. From our extensive experience advocating for and representing consumers in community engagement initiatives, we know that building partnerships requires time, trust, and skill. While we agree that additional requirements can support the desired end-state of new D-SNP EACs—where these committees lead to meaningful improvement of health plan operations and member outcomes through health plan engagement with members in authentic, equitable, and mutually beneficial ways—real-world guidance is needed to help build such partnerships together." Health centers, here is a cool and important opportunity from Vot-ER! It's not huge dollars, but if you work in Black, Brown, Native, and low-income communities and are planning to implement #voterengagement initiatives, you should consider applying for their 2024 Community Civic Engagement Program. It provides hands-on support and small #grants ($5,000 to $10,000) to health centers and look-alikes for implementing voter engagement initiatives using the existing operations of their #communityhealthcenters. "If you want to go fast, go alone; if you want to go far, go together." Early in my career, I worked for Mayor Kay Barnes of City of Kansas City, MO. She is renowned for building coalitions - both formal and informal - to transform KC's urban core. Leaders with this ability always impress me, especially when they bring non-traditional partners to the table. A recent Public Affairs Council piece by Francoise Stovall reminds us that building a successful coalition requires innovative thinking and a willingness to look beyond traditional allies. Here are three key insights I took away: 🌈 𝗗𝗶𝘃𝗲𝗿𝘀𝗲 𝗣𝗮𝗿𝘁𝗻𝗲𝗿𝘀𝗵𝗶𝗽𝘀 𝗗𝗿𝗶𝘃𝗲 𝗜𝗺𝗽𝗮𝗰𝘁: Traditional partnerships are essential, but the real magic happens when you engage non-traditional allies. By collaborating with a broader spectrum of organizations, you can amplify your message and reach new audiences. Think outside the box and seek out partners who share your values but may not be in your immediate network. 🌳 𝗖𝗼𝗺𝗺𝘂𝗻𝗶𝘁𝘆-𝗖𝗲𝗻𝘁𝗿𝗶𝗰 𝗔𝗽𝗽𝗿𝗼𝗮𝗰𝗵𝗲𝘀: Successful coalitions are grounded in the communities they aim to serve. Engage local leaders and grassroots organizations to ensure your initiatives resonate on a deeper level. By prioritizing the needs and voices of the community, you build trust and create more sustainable change. 🤝 𝗟𝗼𝗻𝗴-𝗧𝗲𝗿𝗺 𝗥𝗲𝗹𝗮𝘁𝗶𝗼𝗻𝘀𝗵𝗶𝗽 𝗕𝘂𝗶𝗹𝗱𝗶𝗻𝗴: Building a coalition is not a one-time effort. It's about fostering long-term relationships that can adapt and grow over time. Invest in ongoing communication and collaboration with your partners. This not only strengthens your current initiatives but also lays the groundwork for future projects. Building effective coalitions is about more than just numbers. Asset-based approaches to integrated care can help to put the person at the centre of their care, strengthen community resources, and lead to more creative and effective approaches. These were the conclusions of the International Foundation for Integrated Care (IFIC) Irish webinar which included contributions from Sarah McLaughlin Ewan King Adam Lent & They can also lead to more inclusive offers which enable those from more marginalised communities to benefit from integrated care and to inform service development and investment. Deserving trust is crucial to equitably partner with the communities you engage and to achieve health justice. Remember, though, the process of engagement is as important as the product. This #TipTuesday, check out AAMC Center for Health Justice and their Principles of Trustworthiness. 1️⃣ The community is already educated; that’s why it doesn’t trust you. 2️⃣ You are not the only experts. 3️⃣ Without action, your organizational pledge is only performance. 4️⃣ An office of community engagement is insufficient. 5️⃣ It doesn’t start or end with a community advisory board. 6️⃣ Diversity is more than skin deep. 7️⃣ There’s more than one gay bar, one “Black church,” and one bodega in your community. 8️⃣ Show your work. 9️⃣ If you’re gonna do it, take your time, do it right. 🔟 The project may be over, but the work is not.
