Today marks the start of National Health Center Week! This week recognizes the incredible benefits that Community Health Centers (CHCs) provide to the communities they serve. To kick off this week of celebration, let’s talk about the work that CHCs do with social determinants of health! Social determinants of health are the nonmedical factors that influence health outcomes. They are the conditions in which people are born, grow, work, live, and age, and the wider set of systems shaping the conditions of daily life. Understanding social determinants enables health care providers to transform care with integrated services to meet the needs of their patients and demonstrate the value they bring to communities.
