"During our careers, Dr. Jerry Youkey & I have held the senior-most leadership positions at every level in medicine. That includes a multibillion-dollar hospital system, a medical school, an academic health center, a 2,200 provider medical group & a self-insured organization with more than 16,000 employees that spent approximately $11 million a week for employee healthcare. As such, we have experienced the medical care environment as senior leaders from each of its four components—hospitals, doctors, payors & universities. And looking back, we too heard the noise about how our medical care environment often failed to support patient well-being. It was only after we had stepped away & experienced the medical care environment without a dog in the fight that we saw the global nature of the problem. Now in print! - “Applying the Theory of Guided Participation to Clinical Practice” by RTS Director Emerita Dr. Rana Limbo and Dr. Karen Pridham This is the latest article from these two brilliant researchers who have spent 35 years developing and advancing Dr. Pridham’s recently recognized Theory of Guided Participation (GP). The article illustrates assumptions of GP with clinical cases drawn from GP practice, and it discusses skills and supports needed for the practice and future directions for extending the method. It serves as a companion piece to “Guided Participation for Clinical Practice: A Middle-Range Theory of Teaching and Learning,” which was published last year. Amazing achievement for a major organization, indicating the future of guideline development

Elected to the American Clinical and Climatological Association (ACCA) Thanks FPA for the post - It made me think that in 1884, when the ACCA was founded, doctors resorted to trying to leverage ambient climatic conditions to help with chronic diseases that were weighing heavily on patients. - Fast-forward to 2023, we do know as clinicians treating patients with head-face pain that ambient climatic conditions are also very important, can be critical: think of cold wind, cold in general - both BAD, think of warm - generally experienced as beneficial, with flavors of dry-warm (like US Southwest), and moist-warm (like US South) experienced as beneficial by some. And finally the undeniable impact of changes in barometric pressure where sudden drops are so pernicious for TN patients ... Yet we know so little about mechanisms, which would then enable us to consider rational countermeasures (other than to discuss with the patient to change their residence).
