If you find yourself in the hospital, chances are you are going to be seen by a doctor you’ve never met. That doctor is a Hospitalist – readily available to treat acute care patients. The American Board of Physician Specialists defines Hospitalists as, “…physicians who have dedicated their careers to hospitalized patients.” Simply put, Hospitalists are medical specialists who most often earn a residency in internal, family or pediatric medicine to provide care for adults and children. They encompass diverse skill sets and are specialized in taking care of patients in a hospital setting. “A Hospitalist is somebody who provides comprehensive care and addresses medical needs for whoever walks into the hospital. From headache to heartache a Hospitalist is skilled to provide general medical care,” says Dr. Tooba Anum, Hospitalist with Chatham Hospitalists, who sees patients at St. Joseph’s Hospital. “We are the basic driver of clinical care, and when deemed necessary, we incorporate multi-disciplinary approach by consulting specialists.” The trend of Hospitalists started about 25 years ago. St. Joseph’s/Candler contracts with Chatham Hospitalists, LLC, which formed in 2005 in Savannah. Currently, there are approximately 30 Hospitalists between St. Joseph’s Hospital and Candler Hospital that are staffed around the clock to care for patients. There are also hospital-dedicated nurse practitioners that see patients. More at https://lnkd.in/eaiYjSgb #hospitalist #hospitalists #healthcare #hospitals #medicalcare

Transitional Care Management What is Transitional Care Management (TCM)? Transitional Care Management (TCM) services address the hand-off period between the inpatient and community setting. After a hospitalization or other inpatient facility stay (e.g., in a skilled nursing facility), the patient may be dealing with a medical crisis, new diagnosis, or change in medication therapy. Family physicians often manage their patients’ transitional care. TCM Coding The two CPT codes used to report TCM services are: CPT code 99495 – moderate medical complexity requiring a face-to-face visit within 14 days of discharge CPT code 99496 – high medical complexity requiring a face-to-face visit within seven days of discharge Requirements and Components for TCM -Contact the beneficiary or caregiver within two business days following a discharge. -The contact may be via telephone, email, or a face-to-face visit. Attempts to communicate should continue after the first two attempts in the required business days until successful. -Conduct a follow-up visit within 7 or 14 days of discharge, depending on the complexity of medical decision making involved. The face-to-face visit is part of the TCM service and should not be reported separately. -Medicine reconciliation and management must be furnished no later than the date of the face-to-face visit. -Obtain and review discharge information. -Review the need for diagnostic tests/treatments and/or follow up on pending diagnostic tests/treatments. -Educate the beneficiary, family member, caregiver, and/or guardian. -Establish or re-establish referrals with community providers and services, if necessary. -Assist in scheduling follow-up visits with providers and services, if necessary. Discharge from any of the following: Inpatient acute care hospital Long-term acute care hospital Skilled nursing facility/nursing facility Inpatient rehabilitation facility Hospital observation status or partial hospitalization Health Care Professionals Who May Furnish and Bill TCM Services: Physician (any specialty) Clinical nurse specialist (CNS) Nurse practitioner (NP) Physician assistant (PA) Certified nurse midwife Non-physicians must legally be authorized and qualified to provide TCM services in the state in which the services are furnished. https://lnkd.in/dK5Z54XE #tcm #transitionalmanagementcare #caremanagement #medicalcoding #guidelines #codingtips #codingandbilling

Finding Passion in Wound Care: An Interview with Joan Finding My Niche: Since becoming a registered nurse, I've been drawn to wound care. The intricate process of healing and the opportunity to directly impact patient outcomes deeply resonate with me. Innovation in Wound Care: The field of wound care is constantly evolving, with new technologies and dressings emerging. I'm particularly impressed by UrgoTul's ™ adaptive dressings for bone coverage under negative pressure wound therapy (NPWT). Their ability to promote granulation tissue formation over exposed bone is an asset. A Team Approach: The close-knit team environment fosters a true sense of family at Landmark Hospitals. I appreciate the collaborative spirit and dedication to providing exceptional wound care to every patient. Joan's Guiding Principle: "Your attitude is your choice." Meet Jo, a #TeamCHS Clinical Nurse Consultant on the Acute Medicine Unit. She shared with us intricacies of our patient discharge process. ‘From the very moment a patient is admitted, our discharge planning kicks in. While our goal is to have patients return to their comforting home environment, their safety remains our top priority. This isn’t just a one-day effort – we reassess and plan every single day.’ Patient involvement is key to a smooth discharge. ‘We keep patients informed about their plans. If discharge is possible, we discuss their needs and organise pharmacy, follow-ups, and more. Our aim is to have patients safely on their way home before 10AM.’ Jo also explained ‘criteria lead discharge.’ ‘I This is our way of staying a step ahead. Identifying patients who, come morning, are ready to leave without further medical evaluations. With all set, some can set off as early as 7AM’ Coordinating across departments is crucial. ‘We ensure timely flow and communication, advocating quality and safety of discharge planning from day one,’ Jo added. Finally, Jo's advice for efficient discharge? ‘Build genuine connections with patients. Maintain open channels of communication. It's our holistic approach to care that truly makes a difference.’ #SafeDischarge #PatientSupport #HospitalJourney

As health care continues to face provider shortages, aging populations, and the rising prevalence of chronic diseases, NPs are taking on growing roles as providing important solutions to these challenges. NPs bring a comprehensive skill set to primary care settings where long-term relationships and whole patient health management are key. The extensive training and education empowers NPs to perform a variety of tasks traditionally handled by physicians, from diagnosing and treating various conditions to prescribing medications. Studies have consistently shown positive NP outcomes including high patient satisfaction, reduced hospitalization rates, and overall cost-effectiveness in managing long-term health problems. Now more than ever, nurse practitioners play a growing and vitally important role in the face of our current healthcare challenges. #NursePractitioners #CareForPA #PrimaryCare #FutureOfHealthCare #FullPracticeAuthority #PatientAccess
