'Best practice' is an enemy of innovation in healthcare. When an approach is given the 'best practice' label, it implies no room for improvement. In reality, it may not be the best way to practice at all, and there's always room for improvement.
Best practices are rarely the best — they're mostly just cargo cult common practices. And as more people adopt them, the more mediocre they become. The best is usually what most people aren't willing to do.
