Develops and assist others to develop an individualized Plan of Care, based on the patient's assessment, with mutual, realistic and measurable goals and documents this thoroughly in the patient's record. Coordinates and directs available resources and personnel in developing and following the Plan of Care.

Nurses profoundly impact patient outcomes via direct patient care as well as other work that is indirect patient care. As care is increasingly moved outside of the acute care setting, it is imperative to solve for quantifying the impact of direct and indirect care and determine the best payment models. Unfortunately, money and profit are often the largest motivators in American healthcare. Under the current healthcare system/structures, reimbursement for services is likely one of the only ways in which healthcare at large will recognize the impact of nursing and invest in expanding nursing services both within acute and non-acute settings.
