We are delighted to present to our readers the article titled "The Role of the Nurse in Meeting the Educational Needs for Self-Care in Cachectic Cancer Patients and Their Family Caregivers: A Scoping Review." My 4th APPE rotation, acute care was completed in October 2023. This site was at NYU Langone Health on the main campus. This was the best APPE at one of New York City’s top hospitals with the best preceptor, Dr. John Papadopoulus, director of clinical pharmacy services and pharmacy residency program, critical care pharmacist, author of many publications, and former LIU faculty member. Additionally, I had a great time working with my fellow student, Huma. The rotation block was divided into two weeks of general medicine and three weeks of medical ICU with John. The environment was very welcoming. NYU went above expectations by delivering a surprise welcome package to my house. In summary, this is the top setting to apply clinical pharmacy knowledge and clinical judgment when certain information lacks data, making it a very challenging experience, especially when assessing the patient in every body system from head to toe. Many activities include interdisciplinary rounds, following patients and presenting their care plan, journal clubs, discussions, interventions, drug-information questions, and a final case presentation on hypervirulent Klebsiella pneumoniae. A variety of difficult topics discussed include infectious diseases, shock, critical care medications, acute kidney injury, chronic kidney disease, atrial fibrillation, heart failure, anticoagulation, diabetic ketoacidosis, epilepsy, and alcohol withdrawal. However, after rounding with patients, I got a much better idea of the diseases and each corresponding treatment. John is a remarkable pharmacist, and here are a few takeaway points from his approach. First, his foundation is on pharmacology (his magic words), which is significant because that is the field in which pharmacists are experts. It affects whether the drug can reach the site of action, onset of action, efficacy, toxicity, dosing, adverse effects, and duration of therapy. Another thing to assess is the drug formulation because it affects whether we need to prioritize immediate effects or switch between oral or IV therapies. Finally, it’s crucial to be a lifelong learner by committing time to stay updated by regularly reading literature in pharmacy and medical journals. By doing these things, this is how you would better assess and give the best treatment to your patients. Thus, both professionals and patients would give immediate respect. Overall, I loved the working, learning, and collaborative environment. I thank God for selecting me here to do my acute care APPE given that this was the only block this year that NYU Langone took LIU pharmacy students. It was a wonderful experience working with clinical pharmacists, pharmacy residents, physicians, resident doctors, and medical students. It was a privilege and honor to learn and serve here.

📃Scientific paper: Identification of sepsis in paediatric emergency departments: A scoping review Abstract: AIM: Sepsis is an acute illness associated with significant morbidity and mortality. Early detection and time‐sensitive management of sepsis has been shown to improve outcomes. We report the results of a scoping review to explore methods evaluated for the identification of sepsis in children presenting to emergency departments. METHODS: A systematic literature search was carried out on two databases, Medline and Web of Science, to identify relevant studies published from 1990 to 2022. Data were extracted for age groups including study design, reference standard used for comparison, sepsis identification method evaluated and study quality. RESULTS: A total of 89 studies were identified from the literature search. There was significant heterogeneity in the age groups including study design and reference standards used for evaluating the performance of the sepsis identification methods. There has been a substantial increase in the number of published studies in the last 2 years. CONCLUSION: Our scoping review identifies marked heterogeneity in approaches to identifying sepsis but demonstrates a recent focus of research on patient outcomes. Using appropriate core outcome sets, developing reference standards, monitoring sepsis prevalence via registries and continuously monitoring process measures will provide robust evidence to identify the best performing identification tools and the impact they have on patient‐orientated outcomes.
