Every month, Mosaic Life Care recognizes caregivers who go “above and beyond” and demonstrate one or more of Mosaic’s core values – Diversity and Inclusion, Sacred Trust, Integrity and Life Care – through the In–the–Spotlight Outstanding Performance Award (ITSOPA). What a story! One that we need to share broadly! The Ensign Pennant partnership! We are cut from the same cloth. Parellel core values, both organizations together on a mission to improve the care continuum. To provide life changing service in the post acute space. Our skilled nursing partners hosted some of us this week where we openly talked about what we need to do in order to improve our care across our organizations. Main takeaways- these are true partners, we need to embrace the EPCC partnership and treat them like cluster partners. Learn their buisness, meet often, share best practices, assist with opportunities and collaborate on new and innovative ideas on how to improve our care continuums, but most of all….tell the story. We have had significant growth in both Skilled nursing and HH Hospice we need to ensure our new employees know this relationship! So….tell the story!

Throughout my career, I frequently receive urgent requests for assistance from various sources - ranging from hospital employees, families in search of care for their loved ones, community allies, physicians and peers within our industry. These conversations typically focus on developing care and placement strategies for those in crisis without a plan for long-term or supportive services. Despite these challenges, Skilled Nursing Facilities, (SNFs) are often called upon to overcome these complexities, and they strive diligently to offer supportive care to those in need. Moreover, with hospitals discharging patients sooner, many of whom have increasingly complex clinical needs, our sector's role in providing additional support is more crucial than ever. Achieving the best outcomes requires a collective effort in the most challenging of cases. When we prioritize strengthening our community support and enhancing our alliances across the continuum of care, we acknowledge the vital resources we all provide in support to those with the direst needs.
