Reflecting on my experience as a medical doctor and physiotherapist, a prevailing sentiment among patients is the earnest desire to return home promptly. It underscores a fundamental shift in the way we practice medicine and deliver healthcare in contemporary times. The inclination towards home care and rehabilitation is rooted in the patient's aspiration to convalesce within the familiar confines of their own space, surrounded by loved ones. This paradigm shift invites us to reevaluate and adapt our approaches, acknowledging the therapeutic significance of a patient's home environment. As healthcare professionals, understanding and incorporating this preference into our practices can enhance patient satisfaction and contribute to holistic recovery.

My number one tip for acute episodic spend reduction and long term patient outcome optimization: Invest in a top-notch, high-quality mobility program. While there are some exceptions, patients who are ambulatory at hospital admission should not be discharged at a wheelchair level and unable to do a sit-to-stand transfer. Also, carrying out a mobility program is not usually the role of the physical rehab team in most patient cases. The best day to start or expand a comprehensive mobility program is the day a hospital opens. Coming from a diverse background understanding chronic conditions among diverse populations is important to me to share so others can remain informed as to why culturally-competent education and outreach efforts are important to ensure #careforall. Did you know? 🫁 COPD (chronic obstructive pulmonary disease) In the U.S. affects 7.7% of non-Hispanic whites, 6.7% of non-Hispanic blacks, and 5.2% of Hispanics report having COPD. Racial differences likely reflect higher rates of smoking and exposures to air pollutants and occupational irritants among minority groups. (1) 🫁 Pulmonary hypertension Among Medicare patients, the prevalence of pulmonary hypertension was 291 cases per 100,000 people for Hispanics, 220 for whites, 209 for Asians, and 206 for blacks. It affects women more often than men. (3) Overall, racial minorities and people of lower socioeconomic status tend to have higher rates of chronic lung disease, often related to disproportionate environmental exposures, healthcare access issues, and other social determinants of health.
