🌟 Celebrating National Public Health Week🌟 How do #FQHCs contribute to public health? ➡ Accessible Healthcare: FQHCs serve as vital healthcare hubs, providing a wide range of services including primary care, dental care, behavioral health services, and preventive care. By offering these services in underserved areas, FQHCs bridge the gap in healthcare access, thereby improving health outcomes and reducing health disparities. ➡ Community Engagement: FQHCs actively engage with their communities to understand their unique healthcare needs and preferences. Through outreach programs, health education initiatives, and community partnerships, FQHCs empower individuals to take charge of their health and well-being, fostering a culture of prevention and wellness. ➡ Collaborative Care: FQHCs embrace a holistic approach to healthcare delivery by promoting interdisciplinary collaboration among healthcare professionals. By integrating medical, dental, and behavioral health services under one roof, FQHCs ensure coordinated and patient-centered care, addressing both the physical and mental health needs of individuals and families. ➡ Health Equity: FQHCs are committed to promoting health equity by eliminating barriers to care and addressing the social determinants of health. Through culturally competent care, language assistance services, and sliding fee scales, FQHCs strive to ensure that everyone, regardless of background or circumstance, receives the care they need to thrive. 👍Like, share & comment to help spread awareness about the vital role of FQHCs in advancing public health! #NationalPublicHealthWeek #FQHCs #CHCs #CommunityHealth #awareness

Helping FQHCs, public health departments, and other health nonprofits win more grants, gain greater strategic clarity, and meet community needs. Writer. Facilitator. She/her.
Every August, the National Association of Community Health Centers (NACHC) sponsors National Health Center Week (NHCW) to celebrate and increase awareness of America’s 1,400 community health centers. Given the focus of my work with health centers, of course I love this week! It’s a great opportunity to highlight the commitment and passion of health center staff, board members, and supporters who make it possible to provide quality, comprehensive healthcare services to more than 30 million patients across 14,500 communities annually. The theme this year is “The Roadmap to a Stronger America.” And it kicks off today, with a focus on public housing. Some community health centers receive grant support for the federal Public Housing Primary Care (PHPC) program, a special population designation. The mission of the PHPC program is to provide residents of public housing with increased access to comprehensive primary health care and disease prevention services. In 2021, more than 5.7 million health center patients were served at a location in or immediately accessible to a public housing site. We know housing is health - really, housing is one of the MOST important social determinants of health! Understanding the connection between housing and health enables healthcare providers to transform care with integrated services, meet the needs of their patients, and demonstrate the value they bring to communities and payers. Are you excited about National Health Center Week? And do you agree that housing is health? #nhcw23 #valuechcs #healthcenters #fqhcs #sdoh #socialdeterminantsofhealth #communityhealth #publichousing

To my Health Center friends and colleagues….this picture is from 2015, when we fought tooth and nail for increased HC Funding to avoid going over the funding cliff…the very same scenario we face yet again, later this year. At the time of this post, the House was EXTREMELY divided and there was near gridlock in Congress…sound familiar?! After this post, we went on to advocate in the Senate and won HC funding - averted the cliff, and secured access for an estimated 7 million patients whose care was on the line. But, here’s the most important thing…the power of advocacy was ever present and clear. The hard work, persistence and determination of health center advocates across the country produced an increase AND an extension in funding. SUCCESS was achieved through ADVOCACY, grit, commitment, passion and leadership. We can and will do this again in just a few months. Success is not just within our reach, It is the HC legacy - YOUR legacy! I’ve been honored to support and work along side health centers since 2005 - learning from countless colleagues who brought incredible vision and leadership, and who refused to give up - no matter how tired or unlikely success felt - they fought like hell because there were countless people still in need and their mission was to help. I was then, as I am now, inspired daily to keep doing this work because of their commitment. I post this to remind you of YOUR POWER, the power of your voice and your advocacy and the need to keep going! Advocacy is and always will be the path forward - and when we bring that to focus, the future success and impact we can have is limitless. #advocacy #leadership #innovation #chcs #fqhc #healthequity Advocates for Community Health Vernita Todd, FACHE Kaitlin M McColgan, Craig Kennedy, John Sawyer, Kersten Burns Lausch, Michelle Proser, Abigail Pinkele, Anne R., Michelle Jester, MA, PMP, Susan Jenkins Sumrell, Dawn McKinney, Jana Eubank, Marc Wetherhorn, Dorian W., Elizabeth Kwasnik, MFS, Alex Harris, Association of Clinicians for the Underserved

Strengthening diversity, equity, and inclusion (DEI) and addressing health inequities requires that the voices and perspectives of all patients and families are heard, particularly voices that have been historically marginalized and under-represented in health care. A new resource from the Institute for Patient- and Family-Centered Care (IPFCC) offers real world examples, practical steps, and strategies for improving the diversity of patient and family advisory councils (PFACs) and strengthening the role of patient and family advisors in DEI. Strengthening the Diversity and Role of Patient and Family Advisory Councils (PFACs): Opportunities for Action is available for download at: https://lnkd.in/ejVfrqEG A recording of a related webinar titled More Than Checking a Box: Strengthening DEI in Children’s Hospital PFACs is available at: https://lnkd.in/eMh3GB6R More information about related work is available at: https://lnkd.in/egE7hU-B

future of Primary Health Care #PHC4UHC I had the honor to co-chair the session about the future of Primary Care at todays' WHO conference on Primary Health Care in Astana. Rebecca Rosen, representatives of 4 countries and other participants discussed how we will be able to address the increasing complex and diverse needs of our population and contribute to equity and decreasing existing Health Disparities. We need a person and population centred goal oriented, gender and diversity sensitive approach, delivered through interprofessional collaboration in multi-disciplinary teams. Community health workers are valuable to reach the most vulnerable. We need suffient funding and training and although technology can help ,in the end it is about people: the equal dialogue between the patient and a compassionate health care provider. In this way, strong primary care will ensure that no one will be left behind .
