(ALMOST) ALL WAYS LEAD TO ROME… IN PERIPHERAL ARTERIAL DISEASE (PAD) As the old saying goes there are always many different ways to achieve the same result. To me this is especially true whenever I observe how seemingly similar cases of endovascular interventions for the treatment of PAD can be approached in drastically different ways. Whilst the intended result is the same (=restoration of blood flow to even the most distal segments of the limbs) the different paths of reaching the goal seem to be endless: Here are just a few exemplary yet noteworthy examples: -> Thinking about the destination before even starting the journey: Is a small access sheath used to enable manual compression or is the procedure always ended with a vascular closure device? -> Removing obstacles on your way: Are the clogged arteries debulked via mechanical thrombectomy, regular aspiration or thrombolytic therapy? -> What to leave behind on your journey? If at all… Will tight stenoses be dilated via plain old ballon angioplasty, or the juiced-up version using a drug coated balloon versus stenting, the most permanent of the alternatives? I’m privileged to see cases on a regular basis, being confronted with the shear unlimited number of decisions to be made. This complexity makes it challenging yet rewarding to be involved in these procedures. The more I learn the more I realize how much more there is to learn. Pulmonary fibrosis, an interstitial lung disease, entails continued scarring of lung tissue, affecting breathing and respiratory function progressively. The scarring thickens tissue around and between lung air sacs (alveoli), hampering oxygen flow into the bloodstream, complicating breathing. As time passes, lung tissue stiffens, and causing a gradual decline in breathing ability and worsening respiratory function over time. Pulmonary fibrosis may result from known factors like environmental exposure or medications, or it can be idiopathic, termed idiopathic pulmonary fibrosis (IPF). Though no cure exists for pulmonary fibrosis once lung damage occurs, but early diagnosis and treatment aim to manage symptoms and slow progression.
