ChristianaCare and Beebe Healthcare were rated the #1 and #2 hospitals in the state, while ChristianaCare was rated the #4 hospital for the Philadelphia region as well. Hospital at-home or acute #healthcare at home has been one of the leading trends in care delivery over the past few years. This innovative approach allows healthcare systems to expand their capacities without investing millions in building new brick-and-mortar #hospitals. Some of the nation's top hospitals and healthcare systems are leading the way, pioneering this transformative approach. The results so far have been remarkable, showcasing this model's effectiveness and patient-centric nature. What's particularly intriguing is the temporary waiver that has facilitated this shift. There's a growing sentiment in the industry that this may not be a short-term solution but could become a permanent regulation. 24% of patients who die in the hospital or are transferred to the ICU suffer a diagnostic error. The Institute for Healthcare Quality, Safety and Efficiency received a two-year grant from the Gordon and Betty Moore Foundation to develop a Diagnostic Excellence Capacity Building Program. As part of the Achieving Diagnostic Excellence through Prevention and Teamwork (ADEPT) collaborative, the IHQSE will use the grant funds to create a model for developing and implementing diagnostic excellence programs at 15 hospitals across the country. IHQSE Director, Jeffrey Glasheen, MD, discusses the new grant and how it will support efforts to address and improve diagnostic accuracy across the country. Very interesting news from NHS England this week in Health Service Journal on the formalisation of new national targets for #discharge management and step-down care. A particularly interesting part of this standard is improvement to the way that hospitals capture and manage data on patients' readiness for discharge. This will include a standardised structure of pre-discharge assessments. The Brightfish team here at LOGEX are experts on that, and are ready to assist hospitals with #patientengagement at the bedside to speed up the discharge process for clinically-ready patients.
