🛬 The hub and spoke dispensing consultation outcome has landed! Full response below but the TL;DR: ⏩ Gov. intends to progress with legislation to amend the Human Medicines Regulations 2012 and the Medicines Act 1968, removing the barrier that currently limits hub and spoke models to pharmacies within the same legal entity. ⚙️ Two models proposed for the prescription journey: spoke-hub-spoke and spoke-hub-patient. 💷 Although outside the scope of HMRs, Gov. would expect the spoke, which must be on the NHS pharmaceutical list, to be remunerated for the provision of the dispensing service. The private agreement as to how the hub is paid is to be included in the mandated written arrangement between the spoke and the hub. 🗓️ Subject to parliamentary approval, the proposed legislative amendments are intended to come into force on 1 January 2025. Business Services Organisation (BSO) Family Practitioner Services (FPS) General Pharmaceutical Services for Northern Ireland, Annual Statistics 2023/24 has been published. This annual report, produced in accordance with the Code of Practice for Statistics (https://lnkd.in/dHSvYyqY), contains high level summary information on prescription dispensing and payments in relation to community dispensing (i.e. dispensing by community pharmacies, dispensing doctors and appliance suppliers) in Northern Ireland. Headline results, supported by relevant charts, infographics, commentary, and user guidance are included in the main body of the report. The detailed tables which underpin each of the sections, and which provide further trend, demographic and regional breakdowns at Local Government District (LGD) and Local Commissioning Group (Health Trust) level, are included as Annex Tables. We welcome the Government’s proposal for Lifecycle Adjustment (LCA) within the statutory scheme consultation. It is pro-innovation, rewarding the UK launch of innovative medicines, whilst encouraging launches and competition among off-patent generic and biosimilar medicines that will save the NHS money and avoid worsening the medicine supply risk. The LCA is a potentially bold and creative concept, which we feel provides the best way of finding an acceptable agreement for the whole pharmaceutical industry, while meeting the NHS' financial constraints and ensuring patients gain faster, earlier, and wider access to new and existing treatments. It will be important for us to work through the proposals with the Department of Health and Social Care as to how these can work best work in practice and to mitigate any supply risks in introducing a new way of seeking a rebate from industry. The 340B program is a topic that can elicit a lot of strong opinions among manufacturers, legislators, healthcare providers, and patients. No matter what your role may be in pharma, or outside of our industry, you should want to learn more about this complex and controversial issue. In my role, I spend a lot of time thinking about the 340B drug pricing program. It has huge implications in terms of drug pricing, value creation, access, and, ultimately, the profitability of our brands. However, the program is intended to help vulnerable patient populations through a rebate process, paid for by manufacturers, that can fund healthcare infrastructure, delivery of medical services, and access to medications. These are worthy goals, but is the spirit and intent of this program delivering on its promise? To help us answer this question, Dr. Na-Ri Oh and I talked with William Sarraille, a nationally recognized legal healthcare expert, regulatory consultant, and former partner at Sidley Austin LLP. If you need some good news, then check out what just happened at the Ohio Statehouse! Two bipartisan bills -- focused on increasing access to care across the state -- were introduced in the Ohio House of Representatives and the Ohio Senate. These bills will 1) help to protect the 340B Drug Pricing Program, 2) require pharmaceutical companies to ship their drugs to multiple pharmacies, and 3) ensure that patients can access their medications at a pharmacy of their choice.
