It's a good day to think bigger, do better and be bolder in our work to achieve health equity. Tonight at 7:30pm ET, I'm excited to teach Black & brown clinicians, who have already done the impossible by creating their health practices, to scale to their next level of income & impact through profitable partnerships. The clinicians that are attending are a unique group- they are ready to move beyond their gift of transforming patients and also step into their calling of transforming populations. Funny enough, for my co-founders and I, stepping into that bigger vision was the catalyst for growing the revenue (& profitability), reach and impact of our own practice. 3 years later, with 2 practice locations, 14 more employees & over $1.2 million awarded in grants & contracts, we wouldn't change our decision - to never settle for anything less than our vision. What I enjoyed most at my first conference on primary health care: 🌍 Meeting many PATH colleagues face to face for the first time, getting inspired by their work and having fun together. 🛠️ Learning that community health program should be the backbone and not the appendix of the health system. 🏩 Recognizing the value add of non-public health care services (private, community- and faith-based and corporate). #icphc2023

As Senior Director for Health Equity at the powerhouse national nonprofit Families USA, Staci collaborates with health industry partners, community leaders and health care experts and advocates to advance and win policies that bring resources and justice to underinvested Black, Brown and indigenous communities. On this episode of Power Station Podcast, Staci talks about the threat we all face to losing some of our nation's most impactful policy advancements, including the Affordable Care Act, which Families USA played a significant role in getting over the finish line. As she explains, health equity starts at home, with the state of our housing, the air we breathe, the open space, grocery stores and health care we are, or do not have, access to. Data shows, and communities know, that achieving health equity requires foundational investments in the well-being of all people and communities. Medically fragile patients experiencing homelessness all too often receive either a lower level of care (I.e.-congregate shelter, street outreach, etc…) or a higher level of care (SNF, long term care, etc…) than what is needed to support their illness because of the dearth of available resources to fill the gap. We need more programs like these that fill care gaps and to think more broadly about the impact they have on the health and well-being of our communities.
The recently released evaluation of Project Roomkey showed what is possible when there is political will, cross-sector coordination and collaboration, and person-centered approaches to housing and health care. In this new California Health Care Foundation blog, you can read about two Alameda County Project Roomkey 'sheroes' - Alexis Chettiar and Catherine Hayes. These amazing clinicians leveraged their Roomkey success and learnings to create, in collaboration with Five Keys Schools and Programs and other key partners, transitional and permanent housing for medically frail people.
