For my emergency response peers. Those of you and subsequent employers that are all about certificates... below is what the big three NFPA Standards say... Get off your asses and practice/train. Without it your certificate means nothing. 1.2.6 Fire Fighter I and Fire Fighter II level individuals shall remain current with the general knowledge, skills, and JPRs addressed for each level or position of qualification. 1.2.6* Technical rescue personnel shall remain current with the general knowledge, skills, and JPRs addressed for each level or position of qualification. Technical rescue personnel shall remain current with technical rescue practices and applicable standards and shall demonstrate competency on an annual basis. 1.2.6* Personnel at the scene of a hazardous materials/weapons of mass destruction incident at the levels of awareness, operations, operations mission-specific, hazardous materials technician, and incident commander shall remain current with the general knowledge and skills and JPRs addressed for each level or position of qualification Definition: 3.2.5 Shall. Indicates a mandatory requirement.

On September 4, 2023, at 12:15 hours, W&T Offshore (W&T) reported an injury incident occurred at approximately 09:15 hours on High Island (HI) A 379 B platform Lease G  13808. The W&T Safety Manager reported, “the Injured Person (IP) was found, in the  NBK-1070 Heater Treater Vessel, unconscious, with shallow breathing, weak pulse, and currently was breathing normal and talking, waiting evacuation to shore base for medical evaluation.” Lessee and Subcontractor failure to follow Confined Space Entry Plan. - Failure to conduct Tri-Gas testing and continuous monitoring. - Crew member making entry into vessel without permit. - No Hole Watch identified and posted at hatch opening. - Stop Work Authority not used. - Improper Personal Protective Equipment. #gulfofmexico #confinedspacerescue #confinedspace #offshoreoilandgas #drillingcontractor #oilandgascompanies #oilandgasindustry #bsee

Safety Flash 04/24 available to read / download now > https://imca.me/1I2ltHg We begin in this Safety Flash with a handful of confined space entry incidents: it is important to re-mind ourselves of the basics on confined and enclosed space entry. Firstly, sub-contract personnel were found accessing a confined space in which welding was taking place. Work was stopped and the unauthorized personnel requested to leave the area. Secondly, third-party personnel were found doing hot work at height in a confined space – almost but not quite “breaking every rule in the book”. Again, work was stopped before any harm was done. Thirdly, we pass on a United States’ BSEE alert on incidents that occurred during tank cleaning operations in confined spaces. The fourth event is from the US Coast Guard, a reminder on wire rope integrity management: a wire rope parted, causing a wind turbine nacelle weighing 69 tons to fall 2.2m to deck. Finally we look at an incident in which a CTV (crew transfer vessel) collided with a fixed platform when one CTVs’ engines failed. #IMCAint #safety #HSSE #offshore #marine
