We are really thrilled to present you the results of the TOPOS study. 🤗 It was conducted over for 24 months by the leading European vein centers to determine the safety and efficacy of an oblique common iliac vein stent in patients with post-thrombotic syndrome. Our self-expanding nitinol stent, sinus-Obliquus, was used, which could optionally be extended with our sinus-Venous or sinus-XL Flex. More mapping, less ablation! Yesterday we admitted a middle-aged man without comorbidities to our clinic, who had been previously detected with PSVT on several occasions. During diagnostics, atrial tachycardia easily inducible. We created an activation map of the right atrium with #Octaray catheter, which confirmed the focus on the septal part of the right atrium, 1.5 cm from the His bundle. However, we could not achieve stable catheter position here, so we decided to proceed with retrograde arterial access. We also created an activation map of the aortic root with the #Octaray, which confirmed the focus - unsurprisingly - in the area adjacent to the right atrial focus, in the non-coronary cusp. The first and only RF application with #QDOT micro ablation catheter immediately terminated the tachycardia, which subsequently became non-inducible. The modified Valsalva maneuver (mVm) has been shown to have a higher success rate in converting hemodynamically stable SVT to normal sinus rhythm (NSR) when compared to sVm (REVERT trial). MVm works by increasing venous return and vagal stimulation by laying patients supine and elevating their legs and has been shown to be a simple, safe and cost effective method. #emt #paramedic #ems

This patient underwent #cryoballoon ablation, and #radiofrequency ablation of pulmonary vein isolation (PVI), cavotricuspid isthmus (CTI), posterior wall, and roofline, yet failed to cardiovert him to sinus rhythm. A redo of the same ablations with #PFA using #BostonScientific #FaraPulse technology was performed n got the patient to sinus rhythm, at least for now !
