As we prepare to culminate our Combat Lifesaver training this week, I’m building a comprehensive whole blood transfusion session for both medical and non-medical personnel. I’ve found invaluable resources from the Next Generation Combat Medic website, complementing the wealth of knowledge from Deployed Medicine. On deployment, this is a crucial task for all members of the 4th Security Force Assistance Brigade in our small team layout. In 2014, the Joint Trauma System (JTS) DoD Center of Excellence for Trauma championed whole blood as the optimal resuscitation fluid. The 2018 update from the Committee of Tactical Combat Casualty Care (CoTCCC), noted the preferred resuscitation fluids for casualties experiencing hemorrhagic shock are ranked from most to least favored: 1. Whole blood 2. Plasma, red blood cells (RBCs), and platelets in a 1:1:1 ratio 3. Plasma and RBCs in a 1:1 ratio 4. Plasma or RBCs alone 5. Hextend 6. Crystalloid (Lactated Ringer’s or Plasmalyte-A) Normal Saline is not on this list.
