📝 Final Investigation Report for the Fatal Naphtha Release and Fire at BP-Husky Toledo Refinery Just Released! The U.S. Chemical Safety and Hazard Investigation Board (CSB) issued its final investigation report into the fatal naphtha release and fire at the BP-Husky Toledo Refinery (“BP Toledo Refinery”) in Oregon, Ohio, which occurred on September 20, 2022, and resulted in the death of two BP employees, who were brothers. During an emergency situation at the refinery, liquid naphtha was released from a pressurized vessel, which resulted in a vapor cloud that subsequently ignited, causing a flash fire, which fatally injured the two BP employees. The events on September 20, 2022 resulted in approximately $597 million in property damage at the refinery, including loss of use. Over 23,000 pounds of naphtha were released during the incident. Attached herewith is a comprehensive investigation report which details invaluable safety issues, causes, and recommendations stemmed from the incident. This alert from IMCA (International Marine Contractors Association) highlights the problems when operatives aren't familiar with a task or piece of equipment. And as is usually the case with undesired outcomes, the risk assessment was ineffective and they had completed an MoC to reflect the new style tank. A chemical tank, partly filled with Monoethylene Glycol (MEG), imploded on the back deck of a vessel, resulting in a spillage of around two cubic metres of MEG to the back deck and to sea, and unrepairable damage to the tank itself. There were no injuries. The tank was a 26 cubic metre tank, containing 10 cubic metres of MEG. #industrialaccident #managementofchange #processsafety #processsafetymanagement

"Ineffective Stop Work Authority Policy" One of the causes of the incident was the Refinery's failure to implement a shutdown through the use of Stop Work Authority. Leaders should make it clear that any employee can stop work or shut down the process if they perceive a potentially unsafe situation. The Process Industry must learn from incidents to avoid the recurrence, one of the core responsibilities of leaders.

Vapor Cloud is one of the riskier scenarios when working with flammable fluids. HAZOP must be oriented towards mitigating unexpected and uncontrolled release of this type of chemicals. #processsafety #chemicals #flammable #CSB
