Just going to be blunt for a minute... Everyone keeps pushing the narrative that radiology professionals are not recognized or respected. What we are doing is allowing ourselves to continue to be labeled as "disrespected". That's the narrative we collectively are continuing to speak. We need to stop doing this! Did you know that Radiology has the SAME foundation as one of the 1st largest medical professionals....Let us demonstrate: - We moved from OJT to AA; now having BS, MS, and PhD degrees (advanced levels of education). - We have multiple modalities, specialties, and certifications (levels of advancement within our own profession). - We have our own advanced midlevels; Radiologist Assistant (RA), Nuclear Medicine Advanced Associate (NMAA), and Advanced Practice Radiation Therapist (APRT). We spend more time dismissing/discouraging our own from seeing it as a viable entity and lack advocating to support these pathways. - We have certifications for upper level management and administration (CRA). Many radiology professionals DO hold administrative roles over more than just radiology. What we lack on a larger scale that the 1st largest medical professional does have is self recognition, unity, awareness, and advocacy. Those are all internal (and FREE) things that each of them brings to their profession and orginizations! How can we individually expect others to see the value in what we do when internally we haven't supported our profession at even its basic level of offering. Opposition and disrespect will always exist, but are each of you going to defend ALL of what radiology is, or just the part you work in? It's as simple as a change of mindset and joining in unity with those who are currently doing and speaking the advocacy work for EVERYONE in radiology. What does the scan show? ———————— COLLOID CYST 👨🏽💻Here we have a FLAIR axial sequence of a MRI brain - we know it is FLAIR as fluid is dark, white matter is darker than grey matter and subcutaneous fat is low signal 👨🏽💻Dilatation of both lateral ventricles with a normal calibre 4th ventricle suggests obstruction between these two areas 👨🏽💻Looking more carefully there is a well defined lesion in the region of the foramen of Monro. This is a classic location for a colloid cyst 👨🏽💻These present as high density small lesions on CT and have variable signal characteristics on MRI depending on the state of the fluid within. Treatment depends on the clinical situation. It's Q & A Thursday! Question: Can an interventional radiologist be designated as an authorized user for Y-90? Answer: The role of the AU varies depending upon the institution. In some, an interventional radiology physician serves as the AU. This includes consulting with the patient, ordering and reviewing diagnostic studies as part of the therapy planning, and administering the Y-90 dose. In other institutions, a radiation oncologist or nuclear medicine physician may assume the AU role. They are responsible for reviewing diagnostic data, executing the complex therapy planning, and supervising the Y- 90 dose administration. Code 77790 is assigned by the authorized user (AU) who is responsible for safe handling, receipt, accounting, and storage of the Y-90 dose. A record of the Y-90 dose must be documented and kept in the facility or office where the procedure was performed. However, the NCCI Chapter 9, Section E.12 states: “Supervision and handling of radionuclides is integral to nuclear medicine procedures (eg., CPT codes 78012‐79999). Physicians should not separately report CPT code 77790 (supervision, handling, and loading of radiation source) for this service.” Therefore, when assigning code 79445 for the radioembolization, code 77790 should not be reported for the services provided at the same patient encounter.
