What are the common mistakes new coders make? ✏️ End of the the medical coder's job is as simple as it said it is analyzing Patients' medical records evaluating it and appending the charge codes such as E/M, CPTs, and ICDs. As a rookie medical coder, I entered the training room with minimal knowledge of coding and EMRs. While the training provides basic knowledge, the real challenge arises when you start working. It's crucial to understand what the organization needs, what the project stands for, and what the client requires. The job is as simple as analyzing patients' medical records and appending the correct charge codes. However, many rookies make the mistake of applying everything or nothing. The key is to look at the department's guidelines provided by the client, understand the purpose of the department, and keep all expectations in one place. What is medical coding, how is it performed, and who performs it? Medical coders explain what took place during a patient visit by assigning codes to services and diagnoses. They do this by translating information documented in the medical record into numeric and alphanumeric codes. In the real coding world, they abstract pertinent information from the patient’s medical record and apply their coding guidelines and reimbursement knowledge to receive the best reimbursement possible. Coders need to keep up with coding updates and insurance payment policies, as they can change as often as quarterly. A medical record is what is kept when a patient sees a physician. It includes observations, medical or surgical interventions, diagnostic tests and studies, and treatment outcomes. Either the physician performs the coding, and the coder acts as an auditor, or the coder performs the coding of services provided based on the physician’s documentation. If the coder acts as an auditor, they will verify that the documentation supports the codes assigned by the physician.

As a Philippine Registered Nurse, starting a career as a novice medical coder presents numerous challenges and requires significant adjustments. One of the primary hurdles is coding accurately. New coders must quickly become proficient in medical terminology, anatomy, and the nuances of various coding systems such as ICD-10 and CPT. This learning curve can be steep, and the pressure to avoid errors is immense. Dealing with errors is another common challenge. Mistakes are inevitable for beginners, but they must be addressed promptly to avoid impacting patient care and reimbursement processes. Correcting errors requires attention to detail, patience, and a willingness to learn from mistakes.Adjusting to a new work environment is also a significant aspect of the transition. Novice coders must navigate office dynamics, understand the expectations of supervisors, and integrate into team workflows. Building professional relationships and finding a mentor can be vital in this adjustment period.Lastly, the pressure of meeting deadlines can be daunting. Medical coding is often subject to strict timelines, and new coders must develop efficient work habits to handle the volume and complexity of coding tasks. Balancing speed with accuracy is crucial, and time management skills become essential to manage workload without compromising quality.Overall, while the challenges are substantial, with perseverance and support, novice medical coders can adapt and thrive in their roles, contributing significantly to the healthcare system.
