Patient populations are aging, which often comes with increasing frailty. This doesn’t have to mean reduced quality of life. With comprehensive management of older patient populations, patient-centred Team-Based Care offers a preventative approach to aging. In our latest Bright Spots article, we explore ‘Frailty in Kootenay Boundary: Promoting Patient-Centred Team-Based Care.’ Understanding their region has a comparatively higher proportion of citizens aged 60 to 100 , Dr. Janet Fisher and Dr. Erin Love from the Riverside Family Medicine Clinic in Trail BC, explored strategies for better identification and proactive management of patients with frailty. How can Glaucoma Australia’s SiGHTWiSE program help your patients? This program offers one-on-one emotional and practical support delivered by qualified Orthoptist Educators and trained Health Counsellors to help patients manage the anxiety of receiving a glaucoma diagnosis and support them with their ongoing challenges. In fact, a recent survey into the effectiveness of the program showed that 84% of patients who take part in the SiGHTWiSE Program adhere to their prescribed treatments, and 91% of patients in the Program attend their recommended appointments – both important indicators for management success. In addition, the number of patients surveyed who rated their knowledge of glaucoma and how it is treated as above average or excellent increased from 37% to 51% after receiving education and support through the SiGHTWiSE Program, while those who rated their knowledge as below average, or poor fell from 22% to 6%.
