MBI welcomes the findings of the report The Hidden Waitlist: The Growing Follow-up Backlog, published today by Reform Think Tank, Sebastian Rees, Hashmath Hassan, which estimates that the total number of people currently waiting for follow-up appointments at acute Trusts in England stands at more than 11 million.   “The report highlights the need for greater transparency of the follow-up backlog. Whilst the total size of the list will get headlines, the real question is: how many of the total are overdue? The gaps in information are concerning, but perhaps inevitable given the polarised focus on one set of patients over another. We fully support the report’s main recommendation to report on these patients regularly.   It is only through full transparency of patient demand and visibility of all waiting list activity that hospitals and the NHS can properly inform patient safety, recovery planning and direct precious resource.   With patients increasingly suffering negative outcomes from long waits or missed follow-up appointments, it’s essential that the NHS has the data to prioritise all of its patients, not just RTT. We need to understand the competing demands between these lists and focus on patient care and risk. Transparency and better data are the basis for doing this.   The report raises some key policy issues in how elective care patients are being managed in different ways depending on which list they are on. What is the difference between a planned readmission and an unplanned readmission? The SNFRM evaluates the risk-standardized rate of unplanned hospital readmissions. To determine if a hospital readmission is planned or unplanned, CMS uses a modified version of the Planned Readmission Algorithm originally developed for CMS’s hospital readmission measures. The SNFRM does not count planned readmissions against facilities because they are not indicative of poor quality of care. The SNFRM defines a planned readmission as any nonacute readmission in which one of a set of typically planned procedures or diagnoses occurred.

Admittedly, I am not in the weeds on the formula of HHS's calculations on the disproportionate share hospital payments. But that is not the point of this post. The American Hospital Association is taking aim at the "lack of transparency" in HHS's process in setting the uninsured rate, and accusing it of "cherry picking facts" to support its position. Glass houses. Extremely fragile glass houses. The AHA and other state hospital associations are on the war path to fight transparency laws wherever they can find them. And no, this is not conjecture. I have sat across from these organizations when testifying before numerous public bodies and listened to their arguments against transparency requirements because of the burden and costs associated with such efforts. And cherry-picking? Statements like: "studies show hospital consolidation improves quality and reduces cost" come to mind. Every study, with the exception of one funded by the AHA to support its position, has debunked this statement with hard data and expert analysis. I could go on. Again, whether HHS's position on the uninsured rate is appropriate or not is not really the point. Although I immediately question an organization who has so brazenly taken unsupported positions with such vehemence.

As a paramedic, one of our top priorities is to relieve our patients' pain and discomfort as quickly and effectively as possible. In the prehospital setting, one method we often use to achieve this is administering lidocaine 2% to relieve IO infusion pain. A simple mnemonic I use to remember this method is 2 of 2 over 2 * 2 which stands for "2cc of 2% of lidocaine over 2 minutes, repeated twice." This approach not only helps alleviate the patient's pain swiftly, but it also ensures we are delivering the medication safely and effectively. In the field, every minute counts, and maximizing patient comfort is vital in emergent situations. By effectively managing IO infusion pain with lidocaine, we can help reduce the patient's distress and improve their overall experience. This can also help us establish trust and rapport with our patients, which is essential for providing high-quality prehospital care. By staying informed and implementing best practices such as this mnemonic for lidocaine administration, we can continue to enhance patient care and make a meaningful difference in the lives of those we serve. #paramedic #prehospitalcare #patientcomfort #lidocaineadministration
