At three years old, CooperSurgical's Allison Margolies, MPA was diagnosed with "incurable" chronic myelogenous leukemia. "My parents were told that my disease was terminal and to go home and enjoy the time they had with me. Refusing to accept defeat, they found an experimental program at Boston Children’s Hospital offering bone marrow transplants on leukemia patients like me." Today, Allison is telling her story and sharing how her family building journey was impacted by her treatment, years later. As per Ovarian Cancer Canada (2019), every year, 3,100 Canadians face an ovarian cancer diagnosis. Shockingly, the five-year survival rate for breast cancer is 88%, but for ovarian cancer, it stands at a concerning 44%. Awareness matters. Facing ovarian cancer treatment can be incredibly challenging, especially when it comes to preserving fertility. Whether you're single or in a relationship, thinking about future parenthood while undergoing treatment can add extra stress. OVARIAN CANCER TREATMENT Ovarian cancer treatment may involve removing both ovaries and fallopian tubes, affecting natural pregnancy. However, there are still options available. If the cancer is diagnosed early and only one ovary is affected, it may be possible to keep the uterus and unaffected ovary, allowing for future fertility possibilities. It's crucial to discuss your fertility needs with your medical team before treatment starts. Although preserving fertility may not always be possible, having this discussion can help you plan the most appropriate treatment. Your clinical nurse specialist or consultant can provide you with further fertility counseling and support. WHAT ARE FERTILITY TREATMENT OPTIONS? Fertility treatment options include egg freezing, preserving a woman's eggs for future use, and in vitro fertilization (IVF). IVF allows egg collection before treatment, and if not feasible, donor eggs can be considered. Some areas offer NHS-covered IVF, while private clinics are also available. Other options, such as surrogacy, adoption, or fostering, provide alternative paths to parenthood. Coping with emotions during this journey is essential, and professional counselors, support groups, and specialist nurses can lend a helping hand. Remember, you are not alone in this journey. In a patient with cancer who is pregnant, management decisions must consider any additional risks to the patient due to their pregnant state, and the risks to the fetus. These are often dependent on the gestational age, with limited data to provide information on the risks. Furthermore, if pregnancy termination is desired either due to patient preference or to optimize cancer outcomes, accessibility of reproductive health services may complicate options. Here we outline approaches to manage cancer during pregnancy with a focus on patient preferences, patient and fetal risks, and team-based clinical management.
