This week marks a significant milestone as we celebrate the graduation of our third round of Phase III Cardiac Rehabilitation Maintenance Program (CRMP) participants! From day 1 to completion, witnessing the remarkable growth and resilience of our participants has been nothing short of inspiring. The Phase III CRMP plays a crucial role in the continuation of care after Phase II Cardiac Rehabilitation, offering guidance on exercising independently and confidently post-cardiac event. In this program, we empower individuals to embrace a proactive approach to their health by teaching them not just what to exercise but also how, fostering a sense of self-efficacy and independence. Our aim is to eliminate the need for constant supervision, allowing participants to navigate their fitness journey with confidence. Beyond graduation, we don't just say goodbye—we stay connected and committed. By keeping in touch, we ensure ongoing support, accountability, and encouragement for our participants well past Phase III. It's not just a program; it's a community dedicated to long-term cardiovascular health. It is a sad testament to the state of healthcare in New Yorks State that there are more F rates facilities in New York than anywhere else, and New York does not boast one single A rated facility. I fear the situation will only worsen especially with recent reports that there are more rural hospitals at risk of closure in New York than any other state. More developments on the hospital-at-home front. Many innovative hospitals are going down this road to prepare for the future. It is key to reducing our over-reliance on hospitals and related facilities.
