Hospitals throughout the state are engaged in community-specific programs to improve health and well-being. CMH is no exception. Our Mobile Integrated Health program was recently featured by the Missouri Hospital Association in their 2024 Community Investment Report for our innovative approach to providing care in the community. By extending services beyond the hospital campus, we are enhancing access to primary care and services from community paramedics in patients' homes. Community paramedicine reduces the need for rehospitalization and allows for better coordination of care and community resources to improve health and wellness in the long term. https://lnkd.in/gxAnmpWT

Staff-Led Loving Workflows in Care Delivery 💙 ♦ A Loving Organization - Culture Change ♦ Train the Trainer Facilitator Certification Program for Leaders & Managers ♦ 20 Years Consulting, Training, Speaking in Healthcare
It's rare I watch or listen to a podcast and am inspired. Remembering the word, "inspire" comes from the root, "in spirit", we can say my inspiration has its roots in love, which is exactly what this podcast speaks to: Love as a guiding principle in the leadership, management, and delivery of excellent healthcare. Apurv Gupta, MD, MPH, Stephanie Feals and guest, Erik Anderson of Adventist Health Castle go deep into the practical application of love in creating the excellence in healthcare delivery that wins a National Baldrige Award. But it's so much more than winning this prestigious award. It's about the love that brings together leadership, staff, patients, families and community. Together we're better, yes? The powerful principles that are the foundation of a great organization must now include love. For Adventist Health Castle: Love Matters. For Dr. Gupta and Premier Inc.: A Loving Organization. For 3DHealthcare: Loving Systems. I so love and respect the many kindred spirits who are working so hard to make healthcare better. Having said that, isn't it time, as the song says, to give love a chance. If not now, when? If not us, who? #ALovingOrganization #LoveMatters #3DLovingSystems #3DHealthcare
@erik-anderson-601a8591/, Patient Care Executive at Adventist Health, says from the moment he joined the organization he knew it felt different. He said he felt the sense of Aloha. Treating patients and employees like Ohana - or family - is in their DNA. Erik told us that like with any family, there needs to be trust, transparency, and sometimes even tough love for accountability, which is a two-way street. The team at Adventist Health Castle listens to the community and deeply values that feedback when choosing how to best serve their patients and teams. Their holistic approach includes community-centered events like Christmas tree lightings, speaking to students, and other ways that encourage the community to become a partner in their healthcare. Their commitment to clinical excellence combined with their “Love Matters” campaign lead them to a Baldrige Award win in 2017. To hear more about this remarkable organization, and learn more about how they teach empathy, value vulnerability, and “measure what they treasure”, check out the full interview with Erik: https://lnkd.in/eJe7qwmM Learn more about Adventist Health Castle: https://lnkd.in/e4FzbHVB Topical time codes: 00:43 - Feeling the Aloha 1:51 - Culture within the organization, Ohana 3:55 - Teaching empathy 7:04 - Love Matters campaign and Baldrige Award win 8:30 - How to connect with the community 11:28 - Community engagement 16:15 - Trust, transparency, and tough love 18:52 - Gratitude as part of Love Matters #healthcare #CompassionateCare #communityengagement #particpatorymedicine #aloha #ohana

President at CareAllies – a Cigna company | Focused on connecting care between patients, providers, and payers through value-based care solutions
The infographic from the American Hospital Association included in this article captures the singular role that community hospitals play in the regions they serve. But even in the face of these powerful statistics, there is no way to fully articulate just how great their impact is on the happiness of the people who receive care, but also everyone who knows – and all too often takes for granted – the fact that exceptional care is available. The peace of mind that community hospitals provide cannot be overstated. This is one of the many reasons I believe it is so important that we embrace #valuebasedcare that rewards community hospitals for what they naturally strive to achieve – to help entire communities gain greater health. Connecticut hospitals and health systems are building a healthier state, and their many contributions to the community transcend hospital walls. Hospitals are developing opportunities to drive regional transformative change in community health, care delivery, and health equity. 🏥 Through community benefit programs that encompass a wide range of services, hospitals are collaborating with local organizations to address housing, food insecurity, transportation, diaper insecurity, and employment. Hospitals also provide free and discounted care, subsidize essential health services, and contribute to research -- investments amounting to $3.4 billion in 2022. This #NationalHospitalWeek, learn about some of the many programs that Connecticut hospitals and their partners have united to establish in CHA's "Community Benefit Report," one of three annual publications. The reports highlight hospitals' critical contributions and examine the key challenges facing them. Read the Community Benefit Report: https://lnkd.in/ewxVBCJ3

We are looking forward to connecting with all of our colleagues and making new friends at the 2023 PCA & HCCN Conference! For those attending, we encourage you to stop by and participate in the discussion that will be led by Daniel and Aditya, that will: - Provide a comprehensive understanding of the fundamentals related to savings and losses in value-based contracts with shared risk. - Illustrate the key financial considerations for Federally Qualified Health Centers (FQHCs) entering value-based contracts with risk, including high-priority, upfront investments. - Identify the different internal risk distribution arrangements within a group of FQHCs entering a value-based contract with savings and losses. Here's to a great conference! Camden Coalition has a bit of a cult-like status within complex care models, and the work there is ground-breaking, mission-oriented and thoughtful. Given my admiration for them, the studies showing their lack of demonstrable results point to a simple fact: Care coordination alone, even when implemented well, is insufficient to reduce hospitalizations for this [really, any] complex population of high-need patients. Primary care has proven its value. I am more concerned that a lot of the innovative, Specialty-focused models seem to rely on navigation, coordination and peer supports - without learning the lessons that are tried and true about complex populations. The reality is that these populations actually need a place to go when their needs escalate, a primary care provider to help them and real specialist access/integration for their needs. Let's hope that the next wave of these innovative companies learn these old lessons - these populations deserve it. https://lnkd.in/ecsjcbY2

https://kpsom.link/45KU44A I wish i had come to listen to this conversation Mark Schuster. Your panel shared brilliantly into this primary care discussion. While focused on America and a very different developed health system to Samoa, much of what was shared was applicable to Samoas context. Samoa needs primary care at the core of its health system and public health service with public health supporting and, a clear understanding of what 'public health the specialty', 'primary care' and 'public health service' are. We need a robust primary care infrastructure to deliver on the public health concerns for better patient outcomes. We need health workers trained appropriately in primary care and especially when they are posted to rural health facilities with limited resources. Loved the talanoa on value- based payments / contracts #toyinajahi looking forward to the day of primary care services that puts people and patient outcomes first.

Dear MyCare Supporters, Community Health Centers, like MyCare are now in a finite window until January 19th to secure continual health center funding, otherwise the result will likely be flat funding through nearly all of 2024 (which is equivalent to a 9.3% decrease when accounting for inflation). We need to leverage this window to achieve a long-term extension and increase in health center funding and time is of the essence! Community health centers have faced moments like this many times throughout our history, and it has been people like you stepping up to be an advocate that has made a difference. Our nation's health centers face a critical funding crisis, and we need your expertise and passion to make a difference. With our funding set to expire on January 19, 2024, we are at a pivotal moment. Now is the time to join forces with us and other community health centers to advocate for bipartisan legislation to extend and increase funding for health centers and primary care workforce programs. We encourage you to use this link https://lnkd.in/djEApvDc to easily reach out to your Senators/Representatives and amplify our collective voice. Your advocacy can shape the future of healthcare for millions. Let's act together! #HealthcareAdvocacy #FederalFunding #CommunityHealth #BipartisanAction #MPCA #NACHC #TrustYourCaretoMyCare

This National Hospital Week, we honor America's hospitals, particularly rural and critical-access hospitals that dot the heartland. Working in a rural or critical-access hospital means being part of a close-knit community, and earning trust requires compassion, empathy, and a firm commitment to public health. We admire those who work in these roles to ensure each patient is treated with the same level of quality care they would receive in larger facilities. Staffing for rural and critical-access hospitals is about more than just filling positions; it's about understanding the unique dynamics and needs of communities, and we are honored to be your solution provider in this endeavor.
