A really insightful read into human error, accountability and patient safety. For me, this article touched on historical (and perhaps present) cultures of blame in paramedicine and how this has impeded patient safety. Paramedics need an environment of psychological safety for self-reflection and individual growth to ultimately improve patient safety. Patient safety is at the heart of all we do. This resonates with me working in a foreign system as an international paramedic. Organisational culture reform to encourage self-reporting free of shame or fear of consequences starts with a fair and systems-focused review, rather than focusing on a specific or individual error.
