An incredible, interprofessional group of colleagues are getting together to have a nuanced discussion about the community health worker/peer recovery specialist role. Created to patch the gaps created by our imperfect safety net, CHW/PRS are often individuals who hail from the same communities they serve, hired by health systems to address health related social needs and serve as a bridge between patients and the health system. While being asked to do the impossible job of patching gaps created by structural deficits, they’re usually underpaid and undervalued by health systems. How might health care begin to provide our colleagues in these roles with the support, resources, career pathways and opportunities, and, importantly, compensation that is afforded most of us in the health professions? P is currently detained in a hospital under the Mental Health Act and is prepared for discharge. With a mental health diagnosis and a learning disability, P will likely require social care support to enhance their wellbeing after leaving the hospital. However, there is a conflict: P has one preference, while the nursing team has another, questioning P's ability to make informed decisions regarding their care, living arrangements, and aftercare. P insists that they should have the sole say in these matters. Such scenarios are not uncommon. Nevertheless, the prognosis is optimistic. P can receive support to make decisions about their life after discharge to the extent of their capabilities. In areas where P is deemed to lack decision-making capacity, 'best interest' decisions can be made, incorporating P's views. With my extensive experience working with individuals like P, I commit to understanding P's desires, exploring available options, aiding P in making well-informed choices, and evaluating P's decision-making capacity as necessary. I am equipped to assist P's care team in devising a personalized discharge plan that ensures a seamless transition from hospital to community life.
