The shoulder 'special' test that doubles up as a treatment technique! Today felt like 'shoulder' day in the clinic! 4 of the 6 patients I saw, including 3 new patients all had shoulder pain. They must have all known about my free shoulder pain training webinar tomorrow night! Whilst all today's shoulder pain patient's had different problems... From proximal bicep tendinopathy... Acromio-clavicular joint (ACJ) pain... A lower trapezius issue... And a more straightforward glenohumeral joint (GHJ) problem... Can you see how easy it is to treat the wrong thing? You find a ACJ problem... So you treat the ACJ. Or you diagnose a GHJ or proximal bicep injury, so you focus on that... Even though they do not actually have a problem at that part of the shoulder! The simple 'special' test I used to work out exactly what was going on... And work out in under 30 seconds what was driving their symptoms is not even that 'special.' It is so easy to use and will tell you exactly where to start with each and every patient you see. It can often even double up as a treatment technique!!! Most of the times when someone's been suffering from chronic craniological problems for over 20 years, the road to recovery will be long and difficult. But sometimes you can get lucky. A very, very motivated businessman from Tibet called me regarding 25 years of occipital migraines which onset at the age of 6 years old. He had been suffering from migraines as long as he could remember. He remembers that one of the elders of the small mountain town in which he grew up (elder, or "witch", if I understood correctly), had performed some kind of bone manipulative maneuver on his neck and he was never the same afterwards. He suffered from "just" headaches until his late teenage years. After that, seemingly adherent neck, upper back and shoulder pains were developing, and always triggered in parallel with the headaches. He further explained that they would typically worsen after strenuous use and loading of the arms and shoulders. As I have written about many times, the co-prescence of occipital headaches (or "tension", or "deep pain") along with concomitant shoulder, neck, and upper back pain should immediately trigger your suspicion for thoracic outlet syndrome. In fact, Richard Sanders, the renowned TOS researcher and vascular surgeon wrote that up to 86% of patients with blatant TOS have occipital headaches (Sanders 2008). Furthermore, "arm loading" is a typical trigger in this patient group. And, last but not least, TOS is a common iatrogenic effect of cervical manipulation! We were able to reproduce his symptoms both with the Eden's test as well as with stimulation of the scalenes. Roos' test was positive witin 15 seconds bilaterally, although the pain was predominantly right-sided (IIRC). I started him on propranolol taken 2h before bedtime, as well as various other important implementations for TOS treatment. In a case like this, there is usually no sign of improvement for the first 6-12 months, making it very difficult for the patient to stay motivated. However, already at the first followup there was significant improvement. At the 3 month followup the patient had not had migraine (except when being reckless) and experienced about 90% improvement. The last time I saw him, probably 4 or 5 months in, he still had no headache as long as he was careful. Three important keys in TOS management are to avoid "clenching" habits, maintain shoulder elevation (especially during loading), and to take care that only one single day of worsening should incur after rehabilitation, which is done maximally twice weekly.

The loss of cervical lordosis, often characterized by a straightened cervical spine with no natural curve in the neck, can lead to symptoms such as neck muscle spasms, shoulder and neck pain. Having a healthy neck curve is important as it connects the brain to the rest of the body. It also supports the weight of the head and distributes that weight evenly throughout the spine. When there is a loss of cervical lordosis and the neck becomes unnaturally straight, forward head posture is often introduced, and a condition called ‘military neck’ can develop: the outward appearance of this is a person who appears to be standing at attention. A straightening of the cervical lordosis impacts the biomechanics of the entire spine; it makes it vulnerable to injury, impairs its ability to support the weight of the head, places added pressure on the spinal discs (speeding up disc degeneration), and can cause adverse muscle and joint tension. Loss of Cervical Lordosis Symptoms While forward head posture can be adapted to for a time, over the long term, cumulative effects will be felt throughout the spine, its discs, and the muscles that support it. 1. Vulnerability to Injury 2. Muscle Tension and Pain The muscles in the neck and shoulders have to work harder to support the weight of the head as the curvature that usually facilitates the even distribution of this weight is lost. This can cause the neck and shoulder muscles to feel tight and sore. 3. Disc Degeneration and Weakness When the neck loses its healthy curve, it introduces adverse spinal tension, and this tension increases the pressure on the discs of the spine, causing them to degenerate faster. 4. Postural Changes When you think of the natural soft ‘S’ shape when viewing the spine from the side, you can see how a loss of lordosis at the top of the spine would impact the spine’s overall biomechanics by causing a misalignment. This misalignment can also produce the visual symptom of postural change. When the spine loses its healthy curves, the body responds by putting in bad curves, and if left untreated, bad curves can progress and give the body an overall asymmetrical appearance. When there is a loss of cervical lordosis ant the spine becomes straighter, the forward head posture it introduces can throw off the spine’s overall biomechanics and cause multiple issues. Even if these issues don’t emerge immediately, it’s likely they will later in life. It's good to restore normal cervical curvature as early as possible to prevent from the secondary complications. Excessive use of electronic gadgets is pushing us towards many postural complications. It's good to address your postural issues as early as you can and prevent yourselves from potential medical complications in future.
