Another surgical procedure choice I have always been curious about. ⬇️ Everyone always says ACDFs are a dime a dozen and “you can put anything in there and it will fuse”. Obviously we know that isn’t exactly the case and there are a number of options out there for ACDF cages, plates, etc. My mind always gravitates towards the stand alone vs cage plate constructs. Obviously both have benefits and drawbacks but I wonder what the main decision making process is for surgeons on deciding what style to use, or to utilize a hybrid construct with both types integrated. (I have also seen some surgeons plate over cages with integrated fixation). What makes one style more preferable over the other for you? Joe Auleta with the correct biology and indications as well as adequate disc preparation, you can get either construct to fuse. However there are indications for both, and evidence that stand-alone constructs be limited to incidences of adjacent segment disease. In the study “Comparison of Clinical and Radiographic Outcomes After Standalone Versus Cage and Plate Constructs for Anterior Cervical Discectomy and Fusion” by Fayed et al (2021) they conclude that “Standalone ACDF demonstrates higher, but not statistically significant, revision rates than plate-and-screw constructs, without the hypothesized decreased incidence of dysphagia or hoarseness and without decreased procedure duration or EBL. Surgeons may consider limiting use of these constructs to cases of adjacent segment disease.” Another important factor to consider with either construct is screw length.
