In this operative video, Dr. Cohen demonstrates anastomosis of the STA and MCA for low flow revascularization. The distal end of the donor STA is prepared by removing the surrounding soft tissue. Once the donor and recipient vessels are approximated, a ‘fish mouth’ technique can be used to maximize the surface area available for anastomosis. In this instance, an interrupted suture technique was used, given its ability to prevent stenosis at the level of the anastomosis. Learn more here | https://zurl.co/hF6m.

This patient suffered from a painful bunion deformity with an associated problem called Metatarsus Adductus (MTA). This means that the bones behind the 2nd and 3rd toes, the 2nd and 3rd metatarsals, are also deviated inward in a pathologic position. When this deformity is present it must be corrected in order to successfully correct the bunion. Without correcting the MTA first, the bunion deformity has a significantly greater risk of recurring (coming back) because the bones can't really be lined up in the right anatomic position. In this case we were able to realign the entire midfooot to normal anatomic position, correct the bunion successfully at the unstable joint, and significantly narrow the foot. The exciting thing is that with our technique and patented plating system, the recovery is not any different than "just correcting the bunion", but we are able to give the patient the best chance for a long lasting correction. This patient was protected weightbearing in a boot at 4 days, with transition to shoes around 6 weeks. #bunionsurgery #metatarsusadductus #bunionsurgeryexperts
