Never lose hope 57 years old Patient was admitted elective, he suffered from crescendo Angina in the last 6 Months, CAG showed 1 CAD with CTO RCA ab proximal segment with long Occlusion till distal Segment. The First Recanalisation Attempt : after Using different Wires was Proximal Cup- Puncture successful with ASAHI GAIA III , following very Difficult Balloon Cross, finally was successful with 1.0 mm Sapphire Ballon 🎈, followed by 1.5 mm then 2.0 and 2.5 mm 🎈 , unfortunately still TIMI 0 , Procedure stopped. The Second Attempt after 1 Week: RCA Wiring was successful with Runthrough Extra Floppy , followed by PTCA and 5 DES with good Final Result, however still TIMI 0 in RIVP , we decided currently to Conservative Therapy . #PCI #Cathlab #interventionalcardiology #CTO

Since I moved to the UK I have been using B&L Stellaris with Venturi and now Alcon Centurion with Peristatic. I feel like none of these last two give you the feeling of SPEEP which is a better controlled aspiration without the need of occlusion. Q'Apel Medical Hippo - it’s all about the tip! Below was a case of a M1 occlusion. The MCA bifurcation is variable from patient to patient. Some M1s are long, others short. Some bifurcate, others trifurcation. But since the MCA is occluded in M1 occlusions, you rarely know the underlying anatomy. In the case below, the hippo was advanced into the clot in a relatively straight trajectory… after an initial expansion of the tip, it constrained down and the catheter would not advance further. We turned on aspiration, withdrew, and got TICI 3 recan. Every case has a learning: in this one, after super-imposed the Hippo over the recanalization I learned that once the Hippo ate the thrombus, the pedals followed the vessel anatomy and while the marker band was at the bifurcation, the tip was in M2 - showing me in real time, some of the anatomy that I was otherwise blind to seeing Real time feedback from a flexible, adaptive tip improves my confidence of what’s going on, my understanding on the anatomy I can not see, and therefore safety and effectiveness. It’s all about the tip! #Hippo
