Bar Code Medication Administration (BCMA): Enhancing Patient Safety Medication use continues to rise in Australia, along with the risk of medication-related errors. Alarmingly, 34% of these errors occur during the medication administration phase. Patient safety is paramount, and one of the most effective ways to mitigate risk is through the implementation of BCMA systems. Recent research from the Society of Hospital Pharmacists of Australia (SHPA) highlights that using bar codes on medication unit dose packaging can reduce error rates by an impressive 41%. Source: tinyurl.com/2tyt8h8m Debetrek is committed to improving patient outcomes by sourcing leading edge products for innovative healthcare solutions. Our range of BCMA products are designed to streamline the medication administration process, enhance accuracy, and reduce errors. Navigating Risks in Hospital Drug Therapy Administration: Identifying Emerging Critical Issues. Exploring the evolving challenges in administering drug therapy within hospital settings to enhance patient safety and quality of care. Our study looks at interruptions during medication administration in hospital wards across two units. We ask: Can nurses give medication without interruptions? If not, why, and how can we fix it? We explore ways to reduce interruptions during therapy to prevent patient harm. Our findings highlight the importance of addressing interruptions during medication administration to keep patients safe. https://lnkd.in/g8J-PA6J #PublicHealthJournal #InternationalHealth #HealthIssues #HealthAdvocacy #HealthInnovation

HBOT is becoming increasingly common for off-label use, thanks to the rise in celebrity endorsements and health spa usage - however, not all chambers are the same. Only hard-sided medical grade chambers, capable of sustaining pressure of 1.5-5.0 ATA are approved by the FDA as a medical device and, as such, approved by CMS for patients needing advanced wound care. Learn more about hyperbaric oxygen therapy at hyperbaricaware.com or visit our patient resources for specifics related to the use of low-pressure chambers. Note: All treatment centers that we promote are staffed and supervised by trained physicians. Patient safety is our priority; all centers are equipped with monoplace or multiplace hard-sided chambers at each of our facilities. Chief Optimus @ the company called adherence. | Clinical Trials- endpoints and outcome measures | Evidence based medication adherence metrics | solutions for value based care
https://lnkd.in/gaP3medN Optimal medication adherence assessment during hospitalization emerges as an independent predictor of 30-day hospital readmission rates. Leveraging the MMAS medication adherence tool to identify patients at elevated readmission risk, the real-time availability of this data, and the potential for modifying medication nonadherence make a patient's adherence level a remarkably valuable piece of information. An ongoing research inquiry centers on the practicality and efficacy of executing focused interventions to reduce 30-day readmission rates in patients prospectively recognized as having suboptimal adherence, consequently enhancing a hospital's performance metrics. #medicationadherence #prom #digitalhealth #telehealth

Did you know? Positive patient identification is essential for preventing medical errors and ensuring patient safety. Over 1 million people are treated annually in New Zealand hospitals, errors can lead to harm or even death. Human error is a significant factor in these incidents. 🏥 Labelling systems are key to facilitating positive patient identification, thereby minimising the risk of medical errors. Handwritten records are error-prone and can be challenging to decipher, diverting focus from patient care. #LabellingSolutions #HealthcareLabels #MedicalCoding #ClinicalLabelling #PharmaceuticalLabels #MedicalPackaging #HealthcareIdentification #LabellingTechnology #PatientSafetyLabels #HospitalLabelling #LaboratoryLabels #HealthcareNZ #barcodelabelling #hospitalnewzealand #healthnz #hawkesbay #hawkesbayhealth
