On February 16th, the Alabama Supreme Court ruled that embryos created through in vitro fertilization (IVF) should be considered children. Since then, many IVF clinics have halted their services, fearing legal repercussions. This case was brought by three couples who successfully conceived and gave birth through IVF. Their additional frozen embryos were destroyed when a patient entered the fertility clinic's cryo-preservation unit and “grabbed some of the embryos.” With the new ruling, they can now pursue a wrongful death lawsuit for the loss of the embryos. In this mouse model study, IVF exposure not only shows a direct effect on the metabolic health of male offspring, but also significant intergenerational effects. Miaomiao Ban and team at National Research Center for Assisted Reproductive Technology and Reproductive Genetics, Shandong, shows how hepatic fat accumulation was more likely in response to the metabolic consequences of high-fat overfeeding in IVF F2 offspring, and recommend that the metabolic health of the IVF offspring and their descendants should be regularly monitored for the early prevention of metabolically related diseases. I agree this is a great mental gymnastics to address the needs and begin a discussion. I also understand that change often needs to be gradual. Many people are afraid, uncomfortable with radical changes (despite ‘radical’ originating from ‘roots,’ addressing root problems). I noticed a contradiction in the consensus. On one hand, it’s established that hysterectomies and surgeries by OBGYNs are more technically challenging than egg retrievals. Yet, the consensus (assuming some authors may have dissented) concluded: ‘Reproductive Endocrinologists (REIs) should perform transvaginal oocyte retrievals and embryo transfers, and this committee does not support non-REI providers performing these procedures.’ I commend and support the committee’s efforts to maintain Board Certified REIs at the top and set the rules in our field. However, I respectfully disagree with the assertion that properly trained OBGYNs cannot safely and effectively perform egg retrievals and embryo transfers. OBGYNs already perform transvaginal ovarian cyst aspirations, IUIs, and insertions of IUDs. Therefore, I fail to see the logic behind the committee's conclusion that prevents OBGYNs from assisting us more in fulfilling society's needs.
