Point-of-care Ultrasound, #POCUS, performed by trained emergency physicians on patients suspected of Deep Vein Thrombosis (#DVT) halves patients’ length of stay ! 50% reduction 🚀 This is a big think for overcrowded ER and could incentivize ER doc and nurses to get trained for POCUS and implement this for their patients. Training isn’t an easy task and I strongly believe #AI software, particularly if embedded in the POCUS device, will be a key factor to facilitate the training and the performance of these exams by ER care providers (who are not traditionally performing these exams) It’s by the way already happening with ThinkSono, a UK #startup developing software to train and assist care providers for DVT POCUS diagnosis at the point of care. “Triage in Emergency, Combat, Acute Care and Disaster Environments”                              Evolution of the clinical discipline of Triage* “Triage, the sorting of patients according to the severity of their injuries and the need for urgent surgery, was a concept developed by Dr. Larrey, a military surgeon in Napoleon’s army…..Triage in civilian practice first became a serious issue with the development of dialysis for chronic renal failure in the 1960’s and the problem of allocation of this scarce and very expensive treatment. With new developments in organ transplantation and technology, it continues to be an issue today.” Blagg, C, Triage from Napoleon’s time on the battlefield to the present day. The Bulletin of King County Medical Society 2010: 1:10-25.                                           Our animal spirits avoid discipline, whether self imposed in our life and work or imposed upon us from the outside – as in punishment for perceived wrong doing. But the needs of our times require the Clinical Discipline of Triage. But, just what is Triage? We hear the word used all the time in many different contexts – usually associated with emergency clinical practices and response to disasters. But, the public was first made conscious of “Triage” as a discipline in civilian Surgery when profiled in the 1965 NBC documentary, “Who Lives? Who Dies?”  NBC’s focus was more in the context of the Bioethics of Dr. Belding Scribner’s pioneering work in renal dialysis at The University of Washington Medical Center in Seattle. When new medical technologies, such as organ transplants and kidney dialysis, emerged as practical life-saving treatments, suddenly choices had to be made in United States medicine. There were not enough viable organs or dialysis machines available to all who could be saved by them. So, committees had to be set up in order to determine whose life was worth saving.  “Triage” emphasized the value of a life saved, and Blagg stated that biases in the selection criteria of the committee evolved that triggered a lasting and sometimes strident debate about the ethical propriety of such a ‘triage’ process. Thus, the contemporary public first learned “Triage” as a judgment process for rationing scarce health care resources for the greatest good of the greatest number. Arguably, any one-payer healthcare system has to ration the supply of health care; I know of only one such healthcare system in the Western World – namely Canada. 

New Post: Foxborough paramedics using AI-assisted ultrasounds to better assess patients’ injuries – NBC Boston -The Foxborough Fire Department is embracing the future. Nearly 40 paramedics will now be using artificial intelligence to assess patients’ internal injuries while en route to the hospital. “We’re just getting used to using this new system, so it’ll take some practice,” said Ryan Foley, a Foxborough firefighter and paramedic. According to Foley, their ultrasound... The Foxborough Fire Department is embracing the future. Nearly 40 paramedics will now be using artificial intelligence to assess patients’ internal injuries while en route to the hospital. “We’re just getting used to using this new system, so it’ll take some practice,” said Ryan Foley, a Foxborough firefighter and paramedic. According to Foley, their ultrasound technology will use AI to help perform cardiac and abdominal scans. While the department received an older version of ultrasound technology back in 2018, the updated systems include the first AI-assisted probes in the state. “Instead of having an instructor over your shoulder saying ‘Yah, you’re looking at the right spot, now we have the labeling which helps us to identify what we’re looking for,” explained Deputy Chief of Foxborough Medical Services, Thomas Kenvin. Kenvin confirmed the department received two of the EchoNous Kosmos Torso 1 Ultrasound Probes, and will begin using them the week of March 11. They’ll be utilized during incidents such as high-speed car accidents on the highway, and accidental falls. “We’ve had similar cases where elderly have fallen, where our members have used the probe, not thinking too much, there wasn’t anything outwardly obvious. But using the scan they found there was in fact bleeding in the abdomen, so we would divert from a community hospital to a level one trauma center,” he said. Kenvin added that the technology should help move critical patients into hospital care a little bit quicker. “Especially when we look at hospitals in our part of the state right now, they’re overstaffed and over-burdened with a high-patient census,” he said. “So to be able to give that information to a receiving team that much quicker can actually potentially affect better patient outcome, by moving the patient through the system a little bit quicker.” Source link
